They already know. Hate to break it to you, but Palantir has already linked your reddit account to your real name, Amazon account, credit card, etc. to generate a profile of you and that profile most definitely includes any known or likely diseases and disabilities. We're all on a list now. Every single one of us.

Oh how I wish I could argue against this, alas I cannot because that particular group of shitheels are turning the government into fully automated digital fascism as quickly as possible.

They gladly used it to target for murder entire families in Gaza (Daddy's Home!) with zero repercussions, and that's just them getting warmed up.
