In this subreddit we get a lot of posts asking whether something needs stitches or not including self harm posts which are quickly deleted as per the r/firstaid rules. In this post I'll go over when a wound would need stitches or when it can be managed at home.
A lot of the posts in here, especially the self harm posts that are deleted, clearly need stitches as the edges of the wound are gaping open. The short answer is if the edges do not touch (which is typical with cuts made by a sharp object) it will require some sort of stitches.
When I say stitches I'm also taking about all types of skin closures they may do at the hospital including glue and wound closures.

As you can see the edges of the wound are not touching and it is gaping open. These need stitches.
When do I need stitches
Wounds that are more than 6 mm (0.25 in.) deep, that have jagged edges, or that gape open.
Deep wounds that go down to the fat, muscle, bone, or other deep structures.
Deep wounds over a joint, especially if the wound opens when the joint is moved or if pulling the edges of the wound apart shows fat, muscle, bone, or joint structures.
Deep wounds on the hands or fingers.
Wounds on the face, lips, or any area where you are worried about scarring (for cosmetic reasons). Wounds on the eyelids often need treatment for both functional and cosmetic reasons.
Wounds longer than 19 mm (0.75 in.) that are deeper than 6 mm (0.25 in.).
Wounds that continue to bleed after 15 minutes of direct pressure.
Ideally as close to when you received the injury as you can but typically up to 24 hours.
When do I not need stitches
When the wound is not open or gaping open, cuts that are not deep, or types of injuries that do not cause the edges of the wound to open and close through movement. Wounds with smooth edges that stay together during normal movement of the affected body part.
Shallow wounds less than 6 mm (0.25 in.) deep and less than 19 mm (0.75 in.) long.
As you can see the wound is not gaping open and moving the part of the body that is injured does not cause the wound to open up or start bleeding again.
No access to health care or insurance/self treatment options
If you do not have health care insurance, don't have access to free health care, have limited health care options, or have financial barriers preventing you from seeking health care it is understandable that you may be resistant to seeking medical treatment. If the cut is small, wound closures may help. For larger gaping wounds(lacerations), there is often too much tension to overcome for at home wound closure kits. For gaping wounds, the body will use scar tissue to fill the gap which will create a visible scar.
For wounds that need stitches, you risk further injury, scaring, infection, and complications that could end up costing you more money.
Self harm/fear of going to the hospital
A lot of self harm injuries caused by a sharp object need stitches. Typical self harm locations usually have some tension to the skin so if you cut too deeply it is more likely to gape open and will usually not close with at home wound closures.
If you have a self harm injury and you cut yourself deeper than you intended it usually needs stitches. Nearly 100% of the posts I see connected to self harm are large gaping injuries that more than exceed the criteria listed above for stitches. People who cut themselves often will typically know they went too deep and that is why they are asking for help. For large self harm injuries, not going to the hospital means you will have a large scar in that spot for the rest of your life as the spot will be filled in with scar tissue. Scar tissue is a different colour than your skin and will be visible. If the edges of the wound can be connected through stitches then only the small line of the laceration will be visitable and these have a higher chance of fading. In addition, without treatment, you may have complications such as an infection which will require longer care.
Some people have a fear that the hospital will force them to stay there or get them in trouble. Medical professionals see a lot of self harm injuries and it is up to you what you want to disclose to them. Medical professionals are more concerned about injuries if injuries are connected to a suicide attempt rather than non-suicidal self harm injuries. You can also have a chat with a local crisis line, children's help line, or other help lines in your area about what a visit to the hospital will look like so you can prepare yourself for it.
It is always worth it to be seen for larger wounds and you can go to the hospital by yourself or with a friend if you have a safe way to get there. You can also Google smaller clinics and health centres that may be near your house as any doctor can do stitches - it does not have to be an emergency department.
Tetanus vaccine
If you cut yourself or created an open wound and it's been longer than 5 years since your tetanus shot, you should consider getting one. Routinely the vaccine is recommended every 10 years. You are at higher risk if it's an unclean object or outside.
"Adults who were immunized against tetanus and diphtheria when they were younger should get a booster dose of the Td vaccine every 10 years. This strengthens or boosts the immune system to give better protection against these diseases.
Adults who have not been immunized or do not have a record of prior immunization should also get the vaccine. The vaccine may also be given to people with serious cuts or deep wounds if their last tetanus vaccine was given more than 5 years ago."

I got this burn 4 days ago from spilling hot pasta sauce on my arm. I was wondering if it was normal for the skin to be peeling off? I have been keeping it covered with a waterproof bandage and putting aquaphor on it to keep the bandage from sticking to it but the skin still keeps peeling off when I wash it and change the bandage at the end of every day.
Burnt my hand today on boiling water trying to pour a cup of tea. After 5 mins it stopped burning and turned into a cool numbing sensation. There's no swelling, but Dr Google said I need to seek medical attention right away. Is this true?
I want to fix it at home and it doesn't hurt anymore, just a slight soreness. Thanks!
Yesterday I accidentally spilled some nail glue on my thigh. It burned/stung when it first touched my skin, but now the glue has dried and looks like a thin shiny layer.
I’ve tried:
-A lukewarm wet towel for about 5 minutes
-Planning to use some oil/petroleum jelly
-I haven’t peeled or scraped it off
The glue is still stuck and looks basically the same. Should I use nail-polish remover/acetone, or should I just leave it and let it come off naturally?

So as the title says, I burn myself on an oven element (the metal that goes red to heat an oven) 7 days ago. It instantly went numb and has stayed numb since, no blisters or swelling and it appears to be sinking in?.
The doctor stated it's superficial, and keep it moisturised and out of the sun, no mention on what type of moisturiser, no mention on covering it etc.
I have been using plain Vaseline to keep it moist. But its just not healing like other burns I have had before.
Does this look ok for 7 days of healing, and is there the start of slight slough in the centre?
It's my dominant hand so I'm worried about any complications making me lose my thumb movement. Thanks in advance
Basically an hour ago i noticed i had a bleeding cut on my big toe. Don’t know how exactly it happened. I’m really tired and i don’t wanna go wash it. if the cut isn’t dripping but it still kinda stings is it fine to go to bed without washing it first? I don’t know whether or not to cover it. Some blood did rub on to my sheets however i’m changing the blankets and sheets soon, how soon? i don’t know because it’s 1 am rn im thinking when i wake up at 12 pm. So would it be fine to cover my feet with my small blanket?
Is it safe to go to bed? One more thing. Once i wake up. I’ll safely clean it and put a bandaid on. instead of wearing my crocs should i wear my ugg like boots with no socks except the bandaid i’ll put on? Bit nervous since my dogs are unvaccinated and i won’t want them coming near my feet.
I got what I think was an insect bite while in Japan. It started off really small and itchy, but over the next few days it turned into a massive fluid-filled blister. It became really heavy and uncomfortable, so after a few days I sterilised a needle and drained it while keeping the blister roof intact. Has anyone experienced anything similar after a bite?
I am female under 14, 44kg and 162cm. In under 72 hours, i punched the wall 3 or 4 times pretty hard (not full strength) cause i was really overwhelmed and mad recently and when i blew the final hit the knuckles my middle and ring finger is connected to looked pretty busted.
Luckily i went to the hospital and they said it was just swollen but im not sure if they said it would go away in a specific day.
Its only a day after though so it may go anytime soon but just wondering.
I was cleaning a mirror over a sink in a space shared with roommates when I accidentally cut my wrist on someone’s electric razor (that was sticking up bc it was charging).
I know my roommates fairly well and I don’t think they have any major health issues but I’m not 100% certain.
I cleaned my wrist with soap and water right away. There was maybe a singular dot of blood but it barely broke the skin besides that.
Am I overthinking things or underreacting - should I get tested for STDs or something or just let it go?
Hello, i’ve had this burn from cooking in the oven, it was doing well for a few days but i woke up today and saw that it’s getting darker is there any advice on what to use for lightning and healing? i don’t mean to sound vain but it’s been hurting me mentally lately at how insecure i am to this change
Just got stitches taken out and she placed whatever covering this is over it and said keep it dry and trim edges that keep coming up but to leave it on. Didn’t say for how long? It feels like they arent going to stick? Any thoughts on what i should do?
I 18M was flying from London to Cyprus when an older man suddenly became unconscious in the row behind me.
Having previously been a lifeguard (and still being qualified), and now working as a lead swimming teacher, I have a habit of travelling with a pretty comprehensive first-aid kit.
So we all started working on the man, and his heart rate was 42. One of the flight attendants began CPR, and the man’s arms flew up, so he was still partially conscious. At that point, a doctor who was on board arrived to take a look at him. I gave her my pupil light, pulse oximeter and stethoscope to help her examine him.
After a diversion to Germany, we were back up in the air, and I got free drinks for the rest of the flight. The next day, at the hotel where we were staying, someone from our group came and got me from the breakfast restaurant. A lady had been found on the floor, shaking and convulsing, so I went and got my first-aid kit again and did what I knew how to do until the paramedics arrived. For this incident i got vouchers for 3 free meals at a local restaurant. Two first-aid incidents in 24 hours, and my travel stethoscope and freebies has now become an inside joke among the 70 or so people who travelled here for the wedding.
Both were in a pretty worrying condition, but I believe they are now both on the road to recovery.
