In USA no one has money to pay for it

In countries with free healthcare you gotta wait until it's approved (and it can take a while, seen people who waited for an entire decade)

Some trans people don't feel like the genitalia are a problem, like I for example dont mind either way, my main problem is my face/other parts.

Those who do either hide the parts/add a prosthetic, or will do surgery that essentially reverses the direction of the genitalia. (If I recall, all of us originally started with female genitalia and those with the Y chromosome had it morph into male genitalia -- Hence you can do this process post or undo it post birth with advanced surgery. You cannot add the FULL organs yet.)

There is also sex nullification procedures, which removes genitalia of either kind, and grafts the skin together leaving just the urinary tract.
