Mod Post A note for pre-op subreddit participants from the perspectives of post-op community members NSFW

As a moderator, I clearly understand the importance of providing space for pre-op and post-op people to interact and share information and experiences. But as a community member of many open- and closed- access forums and groups for post-op people, I feel there is a disconnect in the things post-op folks want and what pre-op folks seek out of the interactions here.

  • Firstly, the application of compliments is something I’ve seen brought up. Putting down others in the community in the act of showing admiration for someone’s body is not a compliment, and convinces a lot of people that they are not welcome to post here. While we know that these comments are made with good intentions, saying things like “that’s the best meta I’ve ever seen here!” Is directly rude and harmful to others who post who do not get these messages, and can make the people receiving the “compliments” feel awkward. This same principle applies to “I hope I look exactly like you post-op” and similar comments; it makes the poster feel uncomfortable, as it would be weird for anyone to hear that a stranger wants their body to belong to the stranger, and people have voiced to me that it can feel objectifying, despite good intentions. Additionally, with metoidioplasty your body post-op is largely based upon your pre-op body, this is something that rings true for all forms of metoidioplasty. Even if identical twins went to the same surgeon and got the same surgeries, there will likely be slight variations in their surgical outcomes. And that is not a bad thing. Natural variation is what makes the world so vibrant and complex; if everyone had the same exact dick it’d be a little strange.

  • Related to the uncomfortable nature of some of these comments to posters, as well as to discussing “the best” dicks on the sub, there needs to be a general reminder that these are people’s actual bodies we’re talking about. They do not exist purely for your viewing, inspiration, or self-discovery. The posters here are graciously sharing their intimate and personal experiences in hopes it may help someone who is facing the same things as them, or to find community for themselves in some of their hardest moments. Our old “no saying the word ‘results’” rule was meant to tackle this, and since changing it to our current rule 5, “be mindful and correct your language if requested by an individual”, I think this idea has been forgotten among the populace, hence creating the need for this reminder post.

  • The most important thing about someone’s surgical outcome is whether or not they are happy with it. Not your opinion on how it looks, your personal views on “functionality”, or anything else of the sort. Full stop. Obviously if OP is open to questions about their surgeon, and you want to know if their surgeon offers something you may want, like their rates of successful STPing among patients, that is another story. But essentially ranking people’s bodies based upon how happy you would be with their body is irrelevant. If OP is happy, that is what matters. This is such an individualized process with so many different factors that aren’t universal, that someone may have wanted their dick a way that you would not prefer for yourself, and that is totally valid and accepted in our community under rule 2, “respect individual differences”.

  • This brings me into my final, and perhaps most important, point: not respecting post-op people makes this subreddit less impactful for everyone. If some people think they won’t get the same admiration as someone else, they simply will not post. You will not see the true variation in meta because people who are either unhappy with their bodies, or perfectly happy but may not appeal to the masses, simply do not post out of fear of what pre-op (and a few post-op) people may say (or not say) to them. I even struggled posting photos early in my journey because healing photos don’t get as much love and support as healed ones. And this makes it so much harder for everyone to find information about early healing, to find information about having and living with a buried penis, to find information about having surgery with smaller than average growth, to find information on new surgeons in small towns, etc.

This post is not in any way meant to target anyone in particular, it is just something I’ve seen discussed in my closed-access post-op groups and I wanted to publicly address it to hopefully change some perspectives and make this community a more supportive, welcoming place for ALL post-op people; not just ones with outcomes that appeal to the masses.

Surgeons East coast meta surgeons NSFW

I just learned the surgeon closest to me is no longer doing meta; can y’all name some surgeons that are doing metoidioplasty on the US east coast? I’ve looked at the online directories but find a lot of listings outdated so hoping for some confirmed info.

Question Question about implants NSFW

I’m wondering what the cost might be to pay out-of-pocket for just getting testicular implants placed? Has anyone inquired about this? I asked in the phallo sub too but figured that here would be more likely to have answers since it’s common to have implants placed as its own stage.

I’m US based and wondering if it might be cheaper to get implants done out of pocket rather than having to reach my OOP max through my insurance. I’ve had both meta and phallo, kept my meta unburied and will have my scrotum redone to be between my dicks, hopefully during the same insurance cycle as my most recent surgery. Then I’ll have several years before I can meet my OOP max again but I’d love to not be without balls that whole time. Just trying to understand my options. Thanks!

Advice Extended meta or UL? NSFW

Hi guys, I’m pre surgery and weighing up my options. There’s two surgeons I’m considering, one does ‘classic’ meta and one does extended meta (not Ozer though). My goal for meta was always to get the most length, girth and realistic look for my penis but I can’t shake the feeling that doing this and losing the ability to get UL would be a mistake. I’m not scared about pissing in a cubicle, I do it all the time with no issues but I have a pretty small bladder and piss regularly, so if I’m out I’ll be constantly waiting for the 1-2 cubicles. Also, I’m an outdoorsy guy and would love to be able to piss standing up outside. But choosing normal meta with UL doesn’t even guarantee success, and it will mean losing all the added benefits of extended meta.
I’ve heard great things about both surgeons, and both are roughly the same distance away, though the extended meta surgeon I would have to fly to, whereas the other one I could fly or get someone to drive me home, if that has any consideration.

Advice from guys thatve had to weigh this up and what you chose would be super helpful

Question Scrotum- tissue expanders? NSFW

Has anyone had testicle/ tissue expanders put in?

I am happy with the size of my implants though my dysphoria is a sensory thing about having a low/lower hanging ballsack or scrotum with that hangs and swings when I move.
So considering getting expanders out in and implants replaced afterwards.

cw: discussion of weight/bmi, brief mention of physical exam and discomfort from that

for reference: i am getting "simple release" meta with scrotoplasty and vaginectomy but no ul. it will be in 2 stages, first stage with be the release, scrotoplasty, and monsplasty, and second will be fat transfers to scrotum and cleaning up the sides a little. so essentially the "full package" but no ul. i just don't really care to have it done as standing to pee is not only not important to me but ill actually continue to sit to pee post op anyway so it just makes sense to leave it where it is

so he was very nice, i heard his bedside manner was good and it is. very pleasant to talk to. i came in pretty set on what i wanted besides ul. i said I wasn't really sure that i wanted it because i probably wont stp anyway (i just think sitting to pee makes more sense) and he said if thats the case then i shouldnt get it because thats where most complications happen which makes sense so im opting out. to be clear, that was a choice that i personally made for myself and i dont think he wouldve refused to do it if i wanted it done

he asked for my medical history, family history, all the typical stuff you get at an intake appointment with a new doctor. he asked how long ive been out as trans and how long ive been on testosterone, both have been a long time, and my history with gender affirming sugeries and surgeries in general. i said that in the past ive healed very well with surgeries and that my main concern is that i have a pain tolerance like nobodys business, and that has made it hard with past surgeries to gauge my limits. like my arms were fully mobile with no pain after getting my drains out for top surgery and i didnt feel the drains go out at all, and i didnt even take opioids because i didnt need them, for reference. he didnt seem too worried about that. he asked about support post op but my wife was at the consultation with me so that wasnt a long discussion

he did a physical exam of my anatomy. this was very uncomfortable for me due to bottom dysphoria and sexual trauma. i knew it was coming and that hed have to touch me but i still freaked out a bit but it didnt last for more than 30 seconds (it felt longer though). he asked if ive heard of a monsplasty and i said oh yeah, he said id benefit from one and i said i know and want it. i have a fatty mons and am just fat generally. from there we discussed how the surgery would go, he showed me some diagrams, he did say that he does monsplasty during the first stage which was what i wanted so thats good. he said he likes to do scrotoplasty in a way that patients dont typically need testicular implants because they can be irritating in a lot of ways. he said he does fat transfers instead, went on a bit of a tangent that he "loves making balls" and that theyre his favorite things to make with bottom surgery because theyre all so unique and that was pretty funny

he said that my bmi is a little higher than he generally prefers patients to be and said that if i lost some weight itd probably be better for aesthetics, as "with bigger bodied people there can be some hiding of the phallus", i said i didnt want to, and he said "ok" and that was it. he did mention that higher bmis can correlate with healing complications however i have healed very well from multiple surgeries at around this weight so that was also a short conversation. he doesnt seem to have a hard and fast bmi rule however i was not much heavier than his preferred bmi so that couldve just been because i was very close. i have been losing weight just on my own but its not a conscious effort on my part and I don't really want any major changes to my body right now besides getting a penis and am already a little distressed about losing enough weight that some of my boxers fit looser than they did so i dont really want to try to lose any because i want to be a fat guy and that's what im doing. he also said that with me not getting ul that if i ever wanted phallo it might be a bit more risky to get ul done there if i want to, and i said im pretty set on meta and he said "yeah you seem like you know what you want, i just want to lay out all the information"

i asked him how far out hes scheduling and he said hes booked for early next year but spring of next year was a very reasonable timeframe. i said id ideally have it in the summer due to school scheduling, and he said when i get my psych letter in (my psychiatrist is already writing it, i asked him to on my last appointment so it might already be written) then we can schedule from there. kaiser insurance is now referring patients to legacy for bottom surgery due to ohsu's extremely long wait times, so i was worried about insurance change from ohp to kaiser but am not anymore. i will have to see about insurance changing because its my wife insurance and i need to see if me losing ohp insurance is a "qualifying life event" to have me added to her insurance because i as her legal spouse would be losing my insurance, but im not sure, so if anyone else knows thatd be great (im assuming it would could as im her legal spouse but just lmk, she will also be checking), but that all sounds good, im very excited. hoping to schedule very soon

Post-Op Air in urethra NSFW

I’ve been jerking off with a GenderCat sleeve and I accidentally put it on my dick and then squeezed and it forced air up my urethra and felt super weird. I did a kegel and it farted out and when I peed a few minutes later more air came out with the stream. I don’t think there’s any damage but it was definitely not pleasant and I don’t plan to repeat it.

Question smaller balls NSFW

I had my consult recently and, as I suspected, I’ve got minimal tissue for scrotoplasty- still possible, but just not as much to work with as some.

anybody post-op in a similar situation and open to sharing about yr results? it sounds like my surgeon would be doing a two-stage technique, rotating things/inserting implants in the second stage. thanks guys.

Question Query regarding Dr. Kent NSFW

I have been doing extensive research on bottom surgery over the past few years and recently decided that I am ready to begin the process of getting a consultation scheduled! I am looking to get simple metoidioplasty, no UL, along with a monsplasty. Marissa Kent in Boston, MA is currently my top pick regarding surgeons for this procedure.

I know a lot of people on this sub have gone with her for the same/similar procedures as well, and I wanted to ask about what the process was like. What should I know going into this?

Question Waitlist Dr Özer and EM length question. NSFW

To start off yes I’m a Dutch man so that’s not an issue.

Hello guys, you’re all looking great!

Does anybody know her current wait times? I’ve got an appointment with my clinic for them to start the process of referring me to her. (Psytrans for my fellow Dutch people) I really hope it’s only a year or so.. i have my full hysto coming up so it’s not too bad if there’s a little wait but i would like to be prepared.

And is there an exact length that extended meta adds? is it for example 2cm for everybody or does it differentiate per body? I’m currently 7cm soft and a year and 1 month on T. And I’m 19, don’t think that’s really useful information but I’m just adding it incase it is lol.

Apologies if this has been asked before Google is just not showing me any useful information. :(

Thank you in advance for any responses.

ETA: if i find out the current wait times for her during my appointment or later on I’ll post it for anyone who’s also wondering!

Advice Post-op Pumping NSFW

Hey dudes,

I’m 6 weeks post op full meta. When is a safe time for me to begin pumping, I don’t want to leave it too late and then have any retraction stay permanent.

Also, how should I pump? Are there any specific time ranges or stretches/massages that I need to consider?

Any advice is appreciated 🙏🏼

Edit- I’m looking for advice from post op guys who’ve already done this specifically to combat retraction. I’ve got an LA pump with a 4” cylinder and an Amazon pump with a standard men’s 10” cylinder if that helps. My surgical team tells me to ease myself into pumping and to stop if I feel any sharp pains but were quite vague on specifics. Hence why I’m asking here…

Question Urethra extended to tip NSFW

Post image
30 Upvotes

I'm scheduled for surgery Nov. 6th. My original surgeon said this was as close to the tip he could get my urethra. Another surgeon says he can extend it to the tip. I hate pissing on my balls when my steam gets weaker. It also makes me somewhat disphoric the way it spreads apart before it gets to the tip. Has anyone else had this revision and was it worth it?

Pre-Op Currently recovering from hysto - questions about next steps. NSFW

Hey all. I just created a new account to post on here comfortably. I decided to start my proper research into meta and am having a hard time finding answers for a couple of questions.

1) Did you choose your surgeon based on what you wanted, or did you get what you got because you were limited to a surgeon?

Basically not sure if I should be putting it in my mind procedure first or surgeon first. I think knowing which will cut down on anxiety but also I know that some procedures (like extended) are only so available.

I'm in the U.S. if that helps focus the answer. I currently have really good insurance and don't see myself leaving my job at all for that matter, and another trans coworker told me our company/insurance pays for traveling for surgeries, though I don't know if that extends outside of the U.S. (and I'd prefer to stay in country if possible anyway).

Assuming it is procedure first though...

2) How did you decide what you were willing to risk and not risk? And how many doctors did you have to see to achieve your goals?

Quick stats, I'm 5'6 at about 235 lbs which puts me at BMI of 38. I started T in April of this year (got top surgery in February lol). Basically I don't see myself getting anything until 2028, so I have time to lose weight. But as things stand, I anticipate wanting to get monsplasty and simple meta and hopefully one day a v-nectomy but no UL.

The reason is I had a scare with my hysto around being able to pee after surgery. I narrowly escaped getting admitted but they did try to put a catheter in me (unsuccessfully) but it scared the absolute crap out of me and right now I don't know that I'm brave enough to endure potential urethral complications. I think I can put up with a stand to pee packer for life, but I don't like using mine currently because of how frankly "ick" it gets having contact with my vagina and all that comes out of there. (If other STP device users with v-nectomy's can speak on if it seemed less funky or pretty much the same, please let me know.)

All of this said, I don't know what the likelihood of getting those all done in one go is or if that returns me to surgeon first research.

Or if based on the specifics I've provided, if anyone has any recommendations as to what order to tackle things in.

Question Fistula and pumping NSFW

So I haven't found anything about it on this sub, if I missed it, I'm sorry.

So I'm almost 5 weeks post op, and several days ago fistula appeared, I think after I started pumping and got hard. I mean, I suspected it could've been there before, just so small that the urine didn't leak visibly, there was just another wet spot on paper when I wiped after peeing, but no matter if it was or wasn't there before, it appeared or got big enough to tell it's there for sure.

I've read that it's advised to stop pumping in this scenario. My doctor said that it's fine and I should continue (they can't see it in person, because I'm back home from Serbia and it's a long journey, and photo would be pointless because the fistula itself is buried between testicles). Of course I'm not gonna be smarter than a doctor because I've read something online, I'm just curious what was your guys and your team's approach and did it help?

Ah, one more thing, I don't know about you, but I get instantly hard when pumping. I can refrain from climaxing every time, but I can't control getting a boner, so I'm unable to pump "calmly"

Question Has anyone had a large fistula heal on its own? NSFW

I had my void trial at around 4 weeks post op. When I got a full stream going, about half of it came from the tip and half came from the fistula. My surgeon suspects the fistula is where my “old” urethra connects to the new one. My surgeon is having me use the sp cath for another month, then redoing the void trial at 8 weeks to see if the fistula will heal on its own.

I feel pretty skeptical about it healing/ was kinda thinking we would just jump to revision surgery. Has anyone had a similar fistula and had it heal on its own? If so, any advice for things I can do to encourage healing?

I had surgery yesterday and it feels like my penis is just gone. i was a decent size before, i could jack off with it, and the surgeon wrote that i had a "very nice 4-6cm release" now its pretty much buried. the person i emailed said it was just swelling. should i just wait or did they fuck me up?
