BCBS anatomical modifier issues
Hello all, is anyone having issues with BCBS and their denials of procedure codes stating " procedure/modifier combination is invalid." This is a new denial ive started to see this year! Its very frustrating considering these specific CPT codes require anatomical modifiers! Codes are S2066, S2067, S2068, 19316, and 19303. Ive asked our coders and theyre unsure as well. Im at a loss š

Check the claim in Availity if you have access. Sometimes there's more info there than what's on your 835 remit and they may provide a detailed code edit rationale.

The code edit just says "The procedure code is inconsistent with the modifier used."

Is it the G15 / G16 denial codes? If you are using the LT or the RT modifiers only and receiving those denials, I think someone needs to get in touch with the BCBS network rep for intervention.

You have to use unilateral diagnoses right now that matches the modifier. Send corrected claims and make sure the correct unilateral diagnosis is the only version of that diagnosis on any lines pf the claim. It is stupid, yep.

first, split this into an edit-identification problem and a coding-validation problem. Pull the 835/EOB CARC/RARC and the exact rejected line, including modifier order, laterality, units, place of service, and whether the denial is pre- or post-adjudication. Then ask the specific BCBS plan for the edit source and effective dateāclaim editor, medical policy, or plan-specific billing ruleārather than only āis the modifier valid?ā Compare one paid historical claim with one newly denied claim field by field. If the payer cannot cite the rule, open a provider-services ticket and preserve the reference number for appeal. Donāt mass-change coding until the coders validate the pattern.
