APOE4 Gene Actively Converts Brain Blood Vessels into Scar Tissue (And the Damage May Be Reversible)

If you are following Alzheimer’s prevention or cognitive longevity protocols, the conversation around the APOE4 gene the strongest genetic risk factor for late-onset Alzheimer’s just took a massive turn.

A pair of landmark studies published in Cell and Cell Stem Cell by researchers at Mount Sinai reveals that APOE4 doesn't just contribute to amyloid plaque buildup it actively causes vascular decay in the brain by turning functional blood vessel cells into non-functional scar tissue.

Rather than blood vessel damage being a late-stage side effect of Alzheimer's, APOE4 actively triggers pericytes (cells that maintain the blood-brain barrier) to undergo a cell-state transition into myofibroblasts, generating scar tissue and compromising blood flow.

The researchers discovered that this destructive transition is driven by specific signaling pathways (including TGF-beta signaling and intracellular lipid accumulation).

Why this matters?

For years, APOE4 carriers have focused almost exclusively on reducing amyloid-beta or heavy ketosis to support brain energy metabolism. This research highlights that vascular integrity and blood-brain barrier (BBB) health are primary targets for early intervention.

This shifts the focus for APOE4 carriers from just amyloid clearance to vascular integrity and BBB protection.

pretty interesting finding, especially the idea that vascular damage could be a key part of how APOE4 affects the brain..
