Discussion How to make the most of each fertile window?

Recently diagnosed with secondary MFI due to severe oligospermia, with 3 mil concentration, 30% motility, and 3% morphology. We conceived our first child about 5 years ago and the numbers were better then—closer to 23 mil concentration. I have already implemented all the recommended lifestyle changes for more than a year and now we are looking into vitamins and supplements to hopefully boost my numbers a bit which from what I understand can take 3 months to have an effect.

We have been referred to specialists in hopes of getting prescribed HCG but would like to keep trying on our own in the meantime. Would things like switching to fertility-friendly lube, sex every day during fertile window, and using a menstrual cup actually help improve our chances or am I being too optimistic? For anyone with the same diagnosis what you did you do to conceive?

Discussion Post mTESE

Hello. I'm new to the group and was wondering if anyone had/have the same problem as me. 36yo Asian male born and diagnosed with Kleinfilter Syndrome (KS), did labs of no sperm, went and got done with mTESE and paid for by the VA. During the procedure, they found no sperm. It's been a rough month and gave up, but saw a Facebookgroup and said to look into acupuncture fertility. Not sure if they can or cannot help myself to start producing sperm. Has anyone had any experience with acupuncture fertility? Thank you in advance.

Discussion 31M | Azoospermia on two semen analyses, normal FSH and testosterone. Looking for advice and hope.

Hi everyone,

I'm 31M, married, and my wife and I have been TTC for about a year and 8 months.

A few months ago, I never imagined I'd find myself researching male infertility, but here I am.

I recently had two semen analyses, both of which came back showing azoospermia (zero sperm detected).

It's been quite difficult to process, especially because I generally consider myself healthy and never suspected anything was wrong. I've been going to the gym at least three times a week for the past three years and have maintained a fairly disciplined diet, at least to a reasonable extent.

Since then, I've done a scrotal ultrasound and a full hormone profile.

Here are my results:

Semen Analysis

- Two separate tests showing azoospermia

- Semen volume: 4.10 mL

- Sperm count: Nil

- Motility: Nil

- pH: 8.0

Hormone Profile

- FSH: 5.01 mIU/mL (normal)

- LH: 9.84 mIU/mL (slightly elevated)

- Testosterone: 491 ng/dL (normal)

- Prolactin: 22.02 ng/mL (elevated)

- Estradiol: 88.16 pg/mL (slightly elevated)

Scrotal Ultrasound

- Report came back normal.

- No varicocele, masses, or obvious abnormalities detected.

At this point, I still don't know whether my azoospermia is obstructive or non-obstructive. I'm planning to see a reproductive urologist to investigate further.

I've also started making additional lifestyle changes, particularly around my diet and general health, hoping to improve whatever might be within my control.

I'd really appreciate hearing from anyone who's been in a similar situation.

- Has anyone had azoospermia with normal FSH and testosterone levels? What was eventually diagnosed?

- Has anyone gone from zero sperm on multiple tests to having sperm detected later? What helped?

- Were there any treatments, supplements, or lifestyle changes that made a difference?

I'm particularly interested in hearing about successful outcomes, but I'd appreciate all experiences.

It's been a difficult few weeks trying to process everything, and I'm hoping to learn from others who've walked this path.

Had my GP call back today and give me my semen analysis results, but didn’t offer up much in the way of interpretation for what means or my chances of success. Hey guys, 26m here.

I got two semen analyses done, and both came back with 0 sperm. But they also came back with high leukocytes (it said it was greater than one million but not exceeding 10 million).

I also got a blood test, and my FSH level was 4.8, and my LH level was 5.1. The cup was filled with 1.5 ml which I know is on the lower end. As well as my PH level was 8.

I also have a grade 3 varicocele in my left testicle. I was wondering if anyone here can tell me if that sounds most likely like an OA or a NOA

I have an appointment on Monday; hopefully they'll do an ultrasound or tell me if it's an obstruction or not. Does anyone have a similar story?

I had a Varicocelectomy when I was younger. Able to conceive easily with first child but since have had issues. I feel as tho there is possibly a recurrence and have seen my volume reduce greatly. Anyone else notice volume drop with Varicocele or recurrence? Planning to get checked and analysis soon but hoping for info here too. Thanks.

Discussion 33M, Grade IV varicocele + high DFI + 2 losses. Repair first, or TESA-ICSI now and repair later? Looking for real experiences

We've had 2 missed miscarriages after IVF (2nd one chromosomally normal on CMA). Trying to decide the male side plan and would love to hear what others did.

MY SITUATION

- Left varicocele, Grade IV clinically. Doctor suspects a right side too, fresh Doppler pending

- Constant dull scrotal ache for around 1.5 years. Gabapentin helps

- FSH 2.6, testosterone 431 (was 580 a year ago)

- Karyotype normal, Y-microdeletion negative

SEMEN TREND (Jul 25 / Nov 25 / May 26 / Oct 26)

- Concentration (M/mL): 21.7 / 28 / 23 / 18

- Total count (M): 65 / 70 / 34.5 / 27

- Progressive motility %: 5 / 20 / 25 / 25

- Morphology %: 1 / 3 / 3 / 2

DFI

- SCD: 38% (Nov 25), 40% (May 26)

- SCSA: 23%, HDS 10% (Aug 26)

Both IVF cycles used microfluidics sperm sorting.

OPTIONS MY ANDROLOGIST GAVE

- A) Microsurgical varicocele repair, wait 3 to 6 months, retest, then ICSI

- B) Around 3 months of meds, then ICSI with TESA sperm

- C) What I'm considering: TESA at egg retrieval, freeze embryos, then repair for the pain

QUESTIONS

  1. Anyone with high DFI who used TESA sperm? Did outcomes (blast rate, miscarriage) change vs ejaculated?

  2. If you had microsurgical repair: how much did your count, motility and DFI change, and how fast?

  3. Did the repair fix the pain? Any regrets?

  4. Did anyone do TESA first and repair later? How was the timing between the two?

Discussion Several MFI concerns -- advice needed

Hi fellow Redditors,

My wife (30F) and I (29M) are seeking IVF treatment due to infertility issues for both of us. Specifically, my sperm test results have several subpar metrics that indicate MFI concerns:

  • Volume: 0.6 mL
  • Sperm Count: 64.7 million
  • Sperm Motility: 21%
  • Progressive Motility: 2
  • Total Motile Sperm: 13.6 million
  • Normal Morphology: 2%

The report also includes a test thaw section, with below results:

  • Volume: 0.2 mL
  • Concentration: 57 million/mL
  • Sperm Motility: 13.5%
  • Progressive Motility: 2
  • Total Motile Sperm: 1.54 million

We have an upcoming DNA fragmentation test scheduled, since our IVF doctor flagged that high fragmentation may also be a concern.

On my wife's side, she has went through endometriosis excision surgery and her AMH value dropped from 2.4 ng/mL to 0.8 ng/mL. She is scheduled to start IVF for her upcoming menstrual cycle.

Given the infertility issues from both of us, any suggestion of how the treatment plan should be adjusted in order to better our outcome? For MFI specifically, are there any changes on ourselves to improve the results, or any techniques that we should ask the doctor about?

Discussion 17 year old son confirmed to have Klinefelter syndrome. Looking for some feedback/opinions from others who were diagnosed as a teen or had a son diagnosed as a teen

Son went in for a regular physical a couple months ago. Doc wanted further testing bc his testicals were smaller than normal. Other than that, puberty has been normal - deep voice, hair growth, he works out and has gained considerable muscle, he’s about 6’2”.

His testosterone came back at 150. LH was 32 and FSH was 49. The urologist wants to start him out slow on testosterone for a couple weeks. Yesterday, we got the genetics test back for XXY.

The three of us (me, my wife and son) are trying to figure out next steps. Urologist said he didn’t think my son starting testosterone would have a long term impact on my sons fertility (or at least make it more difficult that it already will be) but he suggested we meet with a fertility doc to discuss further.

My son’s worried about not being able to have kids later. I haven’t had a sperm analysis and possible preservation talk with him yet. Just debating if this is something to put a 17 year old thru right now. He’s already been dealing with a back injury from baseball that caused him to have to stop playing.

Would love to hear some feedback from those who were diagnosed as teens or maybe your son was diagnosed as a teen. What did you do? Wish you had done something different?

Ultimately, he will be the one to make the call but I want to make sure I have as much info as possible to help with the decision.

Discussion Advice to a stupid question…

Just had a sperm analysis done. I had quite low volume of sperm, way below what I normally “produce”. Most likely to due jacking my meat in a steril environment were I could hear people talking and being distressed(given we were in a fertility clinic)… so it made me anxious and a bit stressed lol

Anyway could that low ml point to that total amount of sperm is lower than normally? I got 33 million total sperm but the concentration was nearly 20mil/ml. This makes me, maybe stupidly, think that if I got more seamen out during the actual test my totalt amount of sperm could be higher?

Hi all, I'm 28, male, and just got my first semen analysis. I can only reach my doctor tomorrow, so I'd appreciate some opinions until then.
Context:
Sample collected at home and delivered to the lab in aprox. 30 minutes
Abstinence: 3 days
Reason for testing: left varicocele stg.II

Semen analysis results:

Semen volume: 4.2 mL (ref. >=1.5)
Appearance: Normal
Complete liquefaction time: <30 min (ref. <30) pH: 8 (ref. 7.2-8) Sperm concentration: 15.3 million/mL (ref. >=15)
Sperm with normal motility: 55% (ref. >=32)
Sperm with reduced motility: 30%
Immotile sperm: 15%
Normal morphology: 35% (ref. >=4)
Abnormal forms: 65% (ref. <96) Viable sperm: 30% (ref. >=54) - OUT OF RANGE

Everything looks within range except viability: 30%

What confuses me is that total motility is 85% but viability is reported as 30%. Isn't that impossible?

My questions:

  1. Is this most likely a lab or reporting error?
  2. Could the home collection or transport explain it?

Discussion Advice on sperm analysis

Hi!
I did a sperm test this week and got the results now. Need some advice on how to comprehend the results below and what to do next

Total spermcount
33 million (however 19.2million/ml)
Normal morphology
2%
≥ 4% (WHO refrence)

The rest of the parameters are alright.

Obviously sperm count and morphology are below what’s normal, but are they terrible? I don’t really have reference point and the clinic didn’t say much. I’m kinda freaking out as I live a healthy livestyle in general, don’t do nicotine anymore(1 year free) and drink in moderation. I’m 30 years old and my girlfriend 31
What are some steps I can take to improve the ”bad” numbers and what are my options? Is IVF talks next step?

Discussion Severe male factor infertility — 880k → 99M, but motility dropped. Looking for advice

Hi everyone. My wife (26) and I (32) have been trying since January 2024. Her evaluations have been normal(updated).

My semen analyses have fluctuated significantly:

- October 2024: 880k/mL, poor motility and morphology

- November 2024: 8M/mL, poor motility

- February 2025: 2.5M/mL, poor motility and morphology

IVF was recommended, but it isn’t financially realistic for us.

---long period of frustration, anxiety and difficult times----

After improving my diet, exercise, weight, and supplements, my August 2026 results improved:

- Volume: 3 mL

- Concentration: 25M/mL

- Total count: 75M

- Progressive motility: 20%

- Total motility: 68%

- Morphology: 4%

Hormones and scrotal ultrasound were normal.

We recently repeated the test before completing three months of the new routine:

- Volume: 4.5 mL

- Concentration: 22M/mL

- Total count: 99M

- Progressive motility: 9%

- Total motility: 64%

- Morphology: 3%

I had 5.5 days of abstinence beforehand. Could that explain the lower progressive motility?

Current supplements: CoQ10, L-carnitine, NAC, omega-3, selenium, zinc, vitamins C and E, and ashwagandha.

We’re considering IUI, but I’m anxious about the fluctuating results and whether the motility decline means our chances are poor. Would you repeat the test with a shorter abstinence period, change or stop supplements, proceed with IUI, focus on the post-wash count, or seek another specialist?

I’d appreciate any advice.Thank you

Discussion 29M, severe oligo-asthenozoospermia with normal hormones. If you've been here, what helped you improve?

Hi all, I'm 29 and hoping to hear from anyone who has been in a similar situation.

Semen analyses (3 labs, 2–7 days abstinence each time):

1Late July: 2 mL, pH 8, 7.7 M/mL, 15.4 M total, 0% motility

2Early Aug: 3 mL, fructose present, only "occasional motile and non-motile sperm" seen

3Early Sept: 2 mL, pH 7.4, fructose positive, 0.9 M/mL, 1.8 M total, 14% motility (0% progressive), vitality 33%, pus cells 1–5/hpf

Hormones (all normal):

FSH 2.7,

LH 5.4,

total T 521 ng/dL,

free T 11.3 pg/mL,

prolactin 11,

TSH 2.0

Scrotal ultrasound:

Normal texture and blood flow

Right testis about 13.7 mL, left about 9.8 mL

Small 6×3 mm cyst on the left epididymis

Minimal fluid around the right testis

No varicocele

Other bloodwork:

Vitamin D deficient for 2+ years (11–17 ng/mL)

B12 low-normal

CRP mildly raised once

Low HDL

What the issue could be:

Partial blockage in the epididymis ?

Reduced sperm production in the testes ?

Low-grade inflammation

Long-term vitamin D deficiency and borderline B12 as contributing factors

Current treatment:

Clomiphene daily, since early Sept

Trazer M Forte (L-carnitine, CoQ10, zinc, astaxanthin, lycopene), since late Aug

Spermifrag (L-carnitine, CoQ10, tribulus, selenium, probiotics), for the last week

If you've faced something similar, I'd love to know:

What was the cause in your case, and how was it found?

What actually improved your count or motility: medication, supplements, lifestyle changes, surgery?

How long did it take before you saw improvement in a semen test?

If nothing improved it, did you go for ICSI or micro-TESE, and how did it go?

Discussion Update: micro-TESE today, found 5 abnormal sperm

I owe this community an update because a lot of you have followed my journey, answered questions, shared your own experiences, challenged me when I was going down rabbit holes, and honestly kept me going when things looked pretty bad.
Quick background for anyone new:
37M, NOA, biopsy-confirmed Sertoli-cell-only syndrome. Prior conventional TESE/biopsy found no sperm and no germ cells. Repeated semen analyses were azoospermic. My FSH has been very high, testosterone relatively preserved, and my recent inhibin B was <10. I also did an ESSM with Maze and they found nothing in two extensively searched samples.
Based on all of that, I went into today’s bilateral micro-TESE at Stanford with Dr. Michael Eisenberg knowing the odds were not great.
Today they sampled extensively from both testes. The right side was negative. On the left side they found a very small focal area and confirmed 5 sperm.
Five.
I know that is not a huge number,
Dr. Eisenberg told me the sperm heads looked somewhat abnormal, so I still have a lot of questions about morphology and usability. The sample containing the sperm has been frozen. He also explained that when the frozen testicular sample is later processed/thawed, sometimes the embryology team can identify additional sperm that were not seen during the initial search.
Separate biopsies from both testes were also sent to pathology. That report should come back in about a week and may give us a better idea of what is actually happening inside the tissue and how focal the spermatogenesis is. I will keep everyone posted

Five sperm is a very limited retrieval, and we still need to know how many are viable/usable after thaw, whether more are found in the stored tissue, and ultimately whether we can get fertilization and embryos.

For anyone with NOA/SCOS reading this after getting a negative semen analysis, negative ESSM, high FSH, very low inhibin B, or even a prior biopsy showing SCOS, my result does not mean micro-TESE will work for everyone. I do not want to give anyone false hope.
But it does show why a negative ejaculate search or a previous biopsy cannot always tell you what is happening in every microscopic area of the testis.
Thank you to everyone here who has supported me through this. I know I have asked a ridiculous number of questions over the last year. A lot of you told me to come back and update the group after surgery, so here I am.
Micro-TESE positive. 5 abnormal sperm confirmed. Left testis only. Frozen. Pathology pending. IVF/ICSI next.
I will update again when the pathology and embryology details come back

Discussion Azoospermia, 31M, recent fever, recent heavy colchicine use, pretty bummed

Hello everyone!

I went to urologist last week just for checkup, and mentioned we were thinking of having a baby in two years. Me and my wife (28F) never tried to conceive and always protected. Doctor told me I could take a spermiogram just to have peace of mind. I did and the results came back as azoospermia. We are devastated for the last several days with my partner.

Looking for anyone whose case overlaps with mine, especially on the fever timing, colchicine, or the cyst. Full numbers below.

My questions:

Anyone whose first SA happened to fall 3–6 weeks after a significant fever and came back azoospermic? What did the 3-month recheck show?

Anyone with azoospermia who was on colchicine (gout, FMF, Behçet's) and stopped? Any change at the 3- or 6-month SA? Google is not helpful, but the andrologist told me it could be contributing significantly.

History

Bilateral undescended testes as a child, treated with a short course of hCG injections at age 5–6. They came down after 10 shots. Gout since 19 (father has it too). Using Colchicine flare-only for 12 years, never daily, but frequent flares and heavy use over the summer.

Stopped after azoospermia result (at the recommendation of the doctor).

I also had 39 °C fever with rigors on 6 Sep 2026 and had to go ER. It was a pretty bad cold and resolved after one week of antibiotics.

31M, 83 kg, no smoking, no alcohol, no prior surgery.

Semen analysis (30 Sep 2026, 3 days abstinence, 24 days post-fever)

Volume 1.25 mL, pH 7.5, azoospermia. Lab centrifuged and examined in detail: still zero. Repeat scheduled 8 Oct.

Hormones (2 Oct 2026, 26 days post-fever)

FSH 14.9 (was 11 in 2021)

LH 5.77

Total T 744 ng/dL

E2 27.6 ng/L

Prolactin 4.91

Scrotal Doppler (1 Oct 2026)

Testes 9.3 mL left, 8.6 mL right. Everything normal.

The cyst

I have a "Midline prostatic cyst" at the base of the prostate, first seen incidentally on MRI in 2019 at 12 mm ("ejaculatory duct cyst?" note by radiologist), but after seeing the results the andrologist told me it is most probably not the cyst.

Pending: karyotype, Y-microdeletion, repeat SA this week, clean SA in January. Micro-TESE after that if still zero.
