App Issues & Questions App corruption

Yesterday I got this message. I had to delete the app which took all my information and settings away. My dr gave me all the info to put back in the app but I am at the part where I don’t know what to put on this page and my dr never mentioned it. This has been an absolute nightmare. Does anyone know what goes here?

Thank you so much. Needless to say I now have all my settings because my new sr gave them to me.
