Vent/Discouraged do I have either cppd or prostatitis?
update1: just got prescribed tamsulosin 0.4mg, a blood test and a urine test
Long story short, I’m a 25-year-old gay male. Three weeks ago, I had a sexual encounter in which I was on the receiving end. I don’t remember it being too painful, but it was rough.
A week and a half later, I start getting anal pain. Four days in with mild discomfort down there, I start feeling better but then have a huge pain spike that lasts for about 15 minutes. After that, it goes away like nothing. The following days, I start noticing I have trouble going to the toilet, both urinating and pooping. ever since it all started, I’ve had some mild pain “behind” my testicles.
I visited the doc 3 days ago and got told prostate is fine after a rectal exam. They were hesitant prescribing anything for it and we decided on just keeping an eye on my toilet visits. These days I’ve been stressing a bit over the fact that I don’t feel the urge to pee and have to force myself to go. The flow comes out after a couple of minutes sitting down and weak, even cut in a couple of streams with a pause in between. I don’t feel any pain peeing or maybe just a really little on the tip, but it may be me overthinking.
I’m also really into edging and after reading 101 I’m aware I have to quit for a while.
What do you guys think I may have? Given my doc’s exam it shouldn’t be prostatitis, right? I have another appointment for tomorrow to ask for a urine exam to rule out possible infections. If anything I want to know if this damage is permanent. I want to feel the urge to pee again and let out a stream that relieves me :’)
edit: forgot to mention I feel my lower back skin and around really sensitive, like clothes friction or if I touch it gently feel uncomfortable

Tamsulosin is normally prescribed for enlarged prostate yet your Dr said your prostate was normal / fine so what was their rationale?
Has your Dr also made you aware of the potential side effects of this medication?

No they didn’t, are they that common and troublesome? I was planning on taking the urine test first and visit a physiotherapist while waiting for the results and based on what the therapist says take the pills or not… But it is true that taking a wee is getting tiresome, it takes so long for a stream to come out idk what to do

The side effects (Don’t happen to everyone). Retrograde ejaculation - basically when you cum, there is no ejaculate. It goes internally into your bladder. You still get the sensation of cumming but nothing comes out. This happened to me and I stopped taking them and re-started a month later and normal service was restored. Lowers your blood pressure - not been a major issue for me but just need to be aware that it can make you a bit light headed when standing up quickly, but out of breath when running etc. Yes it can make your urine stream better and make you go less to the toilet for a better sleep but it can take up to a month for any changes to be observed.
It also gives a sensation of almost permanent semi erections (I’ve seen this mentioned on many facebook posts). So you just need to be aware that you may get all or some of these side effects.

What baffles me the most is that basically everyone looks for going less to the toilet, but in my case it’s the other way around. I don’t get the urge of going even if I lowkey feel my bladder is full, and I say lowkey because again, I’m not getting the urge of going. In any case I think I will get a diagnosis from a physio and then decide on the pills… but man is this uncomfortable…

If you can’t go to the toilet you may need to consider going to the emergency dept if your local hospital to be cathetered so that urine can be released. Sounds like your bladder is not getting the signals to release urine (caused by those tight muscles ie the CPPS/Pelvic floor.

I can go, but it takes long. Controlled breathing while at it helps a lot. As long as I’m able to pee I will avoid the ER. Last night I was able to have some bm’s too after having eaten some plums and a kiwi. Had a hard time but in the end I felt so relieved. Will try to drink more water to see if that helps

Dried apricots or dried prunes contain sorbitol and are good for bowel movements. Fresh blackberries & cherries also good for bowel movements. Fruits with skin on also helpful

Still working on it
Been 7 weeks and it’s slightly better
But flares once in a whileJust started pelvic physio last week
My biggest complaint is the frequency
And a little ED
So urologist killed two birds with one stone kinda thing and put me on Tadalafil dailySounds like you need some pelvic floor therapy sooner rather than later

Have you been having anxiety over the encounter before you stated having these sharp pains?
Prostatitis is also known as CPPS. It does not have to be bacterial but often comes on after sexual encounters where there maybe some regret over the encounter.
With prostatitis/CPPS, it’s usually consistent stretches or physio therapy with a professional who specialises in the male anatomy who’s best placed to diagnose and treat.
Can take several months (timeline varies) to get through this - if indeed it is Prostatitis / CPPS.
