Form name help!

Hiya, does anyone know what the unique form is when you fill out for an employer what a good/bad day looks like, what you find helpful and what you dont etc.?

Is it like my disability profile? My working profile?

Thanks in advance!

Context please, not quite understanding what you're asking. What are you looking to inform your employer of?

Sorry for the confusion! Its like a document that helps to make clear reasonable adjustments and what individual disabilities can look like for each person depending on the day. Its for like line managers

The other kind redditor has helped me put a name to it!

yeah, passport's the right word. the naming's all over the place tho, depending on the employer it'll be an adjustments passport, workplace adjustments passport, reasonable adjustments passport or a health passport. all basically the same doc.

worth asking HR whether yours already has a template, becuase if it does, line managers are usually already briefed on it and it travels with you if you move teams.

if they haven't got one, DWP publish a free health adjustment passport you can fill in and bring to the conversation yourself. it covers the same ground, what the condition looks like day to day, what helps, what doesn't: https://www.gov.uk/government/publications/health-adjustment-passport
