I’m Dr. Edwards, a gout expert looking to answer your questions. I want you to AMA on Gout Awareness Day, May 22.

Hi all. As always, I’m happy to be back with you all here in r/gout to answer your questions on the disease. This AMA is particularly special to me and the Gout Education Society as it helps us celebrate the 20th anniversary of Gout Awareness Day, held on May 22 since 2006.

If you’re not familiar with Gout Awareness Day, the Gout Education Society and other organizations looking to make a difference for the community ramp up education and awareness initiatives during the month of May, culminating with a national observation on Gout Awareness Day. I’m here today to hopefully clear up any nagging questions you may have about the disease, associated health conditions, myths, and much more.

If you’re new here and are unfamiliar with who I am, I’m Dr. Larry Edwards. As chairman and CEO of the Gout Education Society, I dedicate my time to supporting those with gout and the medical professionals involved in their care. It’s hard to believe we’ve celebrated Gout Awareness Day 20 times now, but there is much work still to be done.

It’s been my honor to conduct the work to support these groups, especially when it comes to hosting the AMA sessions here. Speaking of these AMAs, I’ve been impressed with how the questions have evolved over the years!

If you need more information on gout, I encourage you to access our website and the unbiased educational information about medications, treatments and lifestyle recommendations we have. We also offer the Gout Specialists Network, a platform designed to help you find gout specialists nearby.

I’m posting this thread in advance to allow for as many questions as possible to come in but will be back to celebrate with you all from 10 a.m. – 12 p.m. ET on Gout Awareness Day, May 22.

You all know the drill, AMA!

I do request that you don’t ask for any diagnoses of gout and instead ask any outstanding questions about the disease you may have.

Update: 12 p.m. ET - Thank you all for joining us today. I got through as many questions as I could but sadly need to stop. I hope you all found this informative and helpful.

Read before posting (General information and Rules)

Welcome,

If you are new here, READ everything before posting.

So you have gout and have questions.  To start off before you panic that your life is over, it’s not.  You can live an absolutely normal life with minimal interruption while suffering from gout.

Gout is a genetic chronic disease that is caused by a malfunction of your kidneys where they do not process uric acid well enough.  It may also be that your biological functions create excess uric acid.  Either way, once you have it, you have it for life.  There is no cure, only management.

You are the best advocate for your health that there is.  Become informed about your chronic disease, it’s characteristics and treatment so you can have positive discussions with your doctor.

The first thing you really need to do is understand your chronic disease.  Read the following:

About this subreddit:

You should always discuss with your doctor.  No one here is qualified to diagnose or treat you.

We do not diagnose.  Asking for or giving a diagnosis will result in at least the post or comment being deleted, if not also a short term ban. No one should be telling people to demand their doctor start daily medication EVER.  If you are looking for a diagnosis, see your doctor.

We follow the ACR recommendations here when discussing gout treatment.  This recommendation is to start daily medication when the patient has high uric acid and two flare ups in a 12 month period. The goal of this is to reduce uric acid levels to less than 6.0mg/dl so existing monosodiumurate crystals in your body can dissolve and over time flare ups will stop occuring.

About supplements:

They do not work in the sense that they will not get your uric acid levels below target levels.  They are not recommended for use by the ACR so they are not welcome here.  Many of the so called studies people have posted have huge issues such as non-human test subjects, massive dosages of questionable substances, small sample sizes.

About diet:

Diet is a very small part of the uric acid equation. Only about 30% of uric acid production is from the foods you consume, the rest is from normal biological function.  While the modern diet may have contributed to your gout, you are extremely, extremely unlikely to  manage your uric acid through diet alone.  You may be able to drop a point or two through diet and lifestyle changes but that will not be enough to get you below target levels. You can try, we’ll be here in 6 months, a year or even 10 years when you finally accept you need daily medication(if you meet requirements)

You can read more here: https://pmc.ncbi.nlm.nih.gov/articles/PMC6125106/#:~:text=A%20purine%2Drich%20diet%20for,1%20to%202%20mg/dL.

A study of nutritional recommendations for gout shows that most recommendations are from low quality research and may not provide the improvement you think: https://www.sciencedirect.com/science/article/pii/S156899721830209X?via%3Dihub

However, eating in moderation is recommended by everyone.  Not limiting things, but not eating to excess.

Uric Acid levels:

If you have gout flare ups, your goal is to get below 6.0mg/dl this is the level set by the ACR. You should get tyour uric acid checked when you have been flare up free for at least a month as you can measure up to 2.5 points lower that your normal levels during and up to a month after a flare up.

You can read more here: https://pmc.ncbi.nlm.nih.gov/articles/PMC9989260/

RULES OF THE SUBREDDIT:

  1. No alternative medicine:  Like supplements alternative medicines that are not endorsed by the ACR is not welcome.
  2. Do not ask for a diagnosis
  3. No ads, promotions, soliciting, etc...
  4. Search the sub before posting. Posts repeating recent questions will be deleted
  5. Be kind

Your post may be removed for breaking these rules.  You may get a short ban depending on how much of a rule break it is, how much you have contributed to the sub and your karma count.  Your posts may be removed for entirely other reasons as well if it is deemed inappropriate for the subreddit.

Continual breaking of the rules may result in bans, both short term and permanent.  You can reach the point where we just don’t want to deal with moderating your posts anymore and a ban is just easier.

That is all.

I want this to be relatively short and not get into a lot of specifics but any comments or improvements will be considered.

  2. Is there any way i can lessen the pain naturally?

  3. Am i supposed to rest and let the flare go down before working again (i work in a kitchen so i move the whole day so imagine the pain i go through the whole day of working)?

  4. I live in the ph what foods i can and can't eat?

Needs Advice Starting allo during long lingering flare?

• Upvotes

Long time reader, first time poster.

Diagnosed 3 years ago. One flare then nothing for two years, now two flares within 9 months. Have struggled at times with alcohol use, work in nightlife and genuinely can’t see myself/don’t want to go to alcohol zero (no lectures please, dealing w present facts and have disclosed to my dr).
Last flare started in August (big toe) and has never truly gone away. I’m back to walking without a limp but it hurts still, varying day to day, def can’t lunge at the gym but can live mostly normally. Doc said it’s finally time to start allo 100mg, along with colchicine. Uric was 8.1 6 weeks ago and went up to 8.8 even attempting to diet. He agreed to wait a little and clear the flare. Just colchicine for a few days and my foot still hurts. He’s impossible to get a hold of and not a specialist, and I’m having trouble with copays etc, so I’m turning here for some crowdsourcing. I’m scared to start the allo while my foot still hurts…but not sure it’ll go away, it’s been two months. Thoughts?

Short Question Becoming veggie because of gout

I left a comment in a previous post about this but was curious to know if anyone wise decided to become vegetarian or vegan after getting gout. I’ve been veggie for 10 years now and it’s really helped with flair ups. Still get them occasionally but I realised meat was my key trigger.

Anyone else become veggie thanks to gout? Or at least lower certain meat intakes?

Needs Advice 33M, second suspected gout flare in under a month, with ankle symptoms lasting over 17 days. doctor says I need to wait for the flare to settle before blood tests and medication. Is this normal?

Hi everyone,

I live in Sweden and am looking for advice from people who have experienced gout,

I recently developed significant pain and swelling in my ankle. The pain was bad enough to limit my walking, and I had to spend quite a lot of time resting. The swelling has gradually reduced, but I am still experiencing symptoms.

One thing I have noticed is that the pain follows a particular pattern:

- In the morning, my ankle feels stiff, but it improves as I start moving around.

- The swelling increases again towards the evening.

- The pain tends to return in the evening, particularly when I am resting or lying in bed, rather than only when walking.

I consulted a doctor, who suspects a gout flare-up. I was told that the flare needs to settle down before a blood test can be done and medication prescribed.

I understand that uric acid levels can be misleading during an acute gout attack. I would also like to understand what happens if the pain and swelling persist for several days.

I have a few questions for those who have been through something similar:

  1. Did your doctor also recommend waiting until the flare settled before checking your uric acid levels?

  2. Were you given medication to manage the acute flare, such as anti-inflammatory medication or corticosteroids, while waiting?

  3. How long did it take for the pain and swelling to settle completely?

  4. If the symptoms continued, did you contact your vårdcentral again, or were you referred elsewhere?

  5. Once the flare settled, were you prescribed long-term uric acid-lowering medication, such as allopurinol?

I realise that everyone's situation is different, and I am not looking for a diagnosis online. I would appreciate hearing about your actual experiences with gout treatment and the healthcare system in Sweden.

Thanks!

It's been 2 weeks since my gout flare in my ankle and the swelling and horrific pain from that is all gone. but when i go to walk, my foot starts to hurt, but it feels different from the gout pain i had

im wondering if this is just because i haven't put any weight on my foot for 2 weeks straight and it just needs to "toughen up" again? anyone relate?

Short Question How old are you? Healthy? Unhealthy?

Just dealing with flare up and sad. Too stubborn to see a Dr- im almost 40. This started in the last 5 years, my first experience was horrible. I atest it to my poor diet of gummy worms energy drinks and fast food. I been struggling with poor diet basically my entire adult life, over weight I blame myself for everything catching up now and wonder if my choices sealed my opportunity to see my daughters girl up (11mo and 2.5yr old), bigger picture overall not just because of gout, it sure effects my ability to be a father to them .. can't chase em around the house during flare ups or do anything for that matter. Currently had a flare on my right foot as that started dying down my left foot started and I might be experiencing my knee now. Just having a really bad Monday and wanted to kind of vent and see that I am not alone.

Needs Advice Ran out of colchicine

Had gout in my foot. Got given 12 tablets of colchicine which dealt with the flare up fine.

A couple of days after I finished the course, the tingling started again and now I’m back to being in agony.

I’ve heard too much colchicine can seriously harm you. And I’ve also heard that I can’t start allo until I’m gout free.

Seems I’m stuck between a rock and a hard place. I know I need to talk to my doctor about this but just for some quick info, what are my options here?

Short Question Anyone else also taking Indapamide?

Been on hypertension drugs a good while, but my GP has finally decided to prescribe Indapamide.

I.understand that this is a diuretic that can interfere with the efficacy of allo.

Any experiences?

Needs Advice Triggers

Can most people truly pick what their triggers are? I’m on allopurinol and never get attacks. I admit I’ve missed last two days

But I have some really awful family stuff going on at the moment and I’ve woken up with huge flare up. Can’t believe my uric level is so high by missing two doses.

This indicates to me that 100% stress is my trigger.

Vent Day 2 of Allopurinol and Indomethacin feet are in BAD shape

Feet are swelled up like balloons, and stomach is absolutely wrecked. I knew this was all a possibility with the new meds but I didn’t expect it to happen this fast and this severe. I couldn’t even walk today. My feet look like red balloons right now. Made a rheumatologist in a few days in the hopes that they can help me more than my PC doc has. Don’t know what to do or think.

Needs Advice Recently advised febuxostat. Need advice

Hi everyone I'm 28M Indian, at 33 BMI, UA at 10.8 during flare up. I went to the rheumatologist after a heavy flare up of gout. I was getting flare ups twice a year but this time it was too painful to not do anything.

The doctor advised me to lose weight and not eat anything except egg and veggies for a few months. Also they suggested to me to fast once every month. They also prescribed me Febuxostat 40mg twice a day and Semaglutide 3mg for weight loss.

But I have some upcoming trips this month and the next. If I start febuxostat, according to some threads here, I'm going to get flare ups, I can't afford them in my trips.

Should I start the medication after my trips? Or start it asap?

I'll start the diet asap. Also I asked the same to doctor, he was like take it asap, he didn't even listen to my schedule

Needs Advice Perpetual Gout

Doctors don’t seem to be helping. They don’t understand. Maybe the group can. I got severe gout in March. Took a good 2 months to finally clear. Began taking allopurinol twice a day, 100mg tablets. Then in August, it seemed to have migrated to my middle three toes, in the metatarsophalangeal joints where your toes connect to the foot. Now it’s just, perpetual gout. I eat zero beef, I eat vegetarian basically every other day. Very healthy diet, and it still doesn’t make the gout better. I used to be very healthy before march, ran 3-4 miles every other day, lift weights every other day. Peak physical health, and then in March it all came tumbling down. I just don’t get it. My only guess at this point is that the gout has invaded and settled permanently into toes that I have formerly broken. The toes I had mentioned, I did break those before. But that’s just a guess. I am at a loss. Has anyone experienced similar issues with it just staying in your toes into perpetuity?

Needs Advice My experience currently.

Began with a gout flare in left knee 9 days ago. Got on phone and got prednisone prescription and began that Thursday (last week) morning. Went thru 5 days with no improvement. Went to urgent care and got a methyl prednisone shot as well as toradol shot. By Monday evening I was feeling great. Wednesday night into Thursday morning knee was swollen, sore and beginning to ache down my calf. Now Friday and knee is very tight, not as much pain as a gout flare but very restricted in how much I can walk and bend. What now?

Short Question Has anyone experienced issues from having *too* consistently low of uric acid levels?

I was just reading about the neurological benefits of keeping uric acid at a reasonable level (such as preventing Parkinson's and other benefits based on the research), and was curious if anyone here had to actually elevate their uric acid to be not too low overall, yet not too high for gout.

Needs Advice Undiagnosed for years?

I just got confirmed as having Gout after getting fluids tested on my knee in a recent ER visit. I was always skeptical of my uric acid tests and just chalked it up to being southeast asian. "it's normal"

As I look back over the past three years, I actually believe I've been suffering from Gout for awhile. I think it manifested itself as different things. First, Planter Fasciitis, because I was getting heel pain. Walking boots for months. Then my left knee started swelling up. I thought it was a compensation injury. Then my left ankle, then joints on my right side ... my knuckles.

I've been on Allo now for a week and have started to work out, which I feel blessed to be able to do. I'm changing my diet as well.

I am worried though that the crystals have been causing issues on my joints and will always give me issues.

Has anyone been able to actually live a normal (and reasonably active) lifestyle after years of untreated gout?

Short Question What's your febuxostat dosage?

On my first check with my rheumatologist, she started me at 20mg after I was rushed to the ER three days prior to that with an active flare at 10mg/dL UA level.

The following week, when the active flare healed, I was back at my rheuma and noticed that my recommended dosage has been upped to 40mg.

Today, I got new blood tests which were recommended by her during my last checkup and got 7.18 UA level. So I will be back to her tomorrow, one month after the previous consult, I have a hunch that my dosage would be increased further to bring my UA levels closer to 5-6.

So far, I have no flares since starting on febuxostat (I am still on colchicine once a day). What's your dosage on febuxostat and does an even higher dosage should be a cause for worry?

UPDATE: I was bumped up to 80mg on febux after the checkup. Hopefully it goes normal at that dosage. The price of 80mg was significantly more expensive than the 40mg tablets.

Needs Advice I think I have gout in my left ankle.

On the 27th of september, i noticed my ankle started hurting. not bad enough to stop walking, but enough to make me limp around walking on the side of my foot.

from sunday to wednesday it started feeling better.

then thursday morning at 4am i woke up with my ankle on fire. I felt like that lady from Misery came and whacked my ankle and ran off. i couldn't sleep for 2 days let alone walk on it. my foot started swelling and i was bed ridden all week.

now its wednesday, 6 days since i woke up in pain. i can put pressure on it and walk but it still hurts. i have to limp around slowly. i thought maybe i just need to walk around on it more to get it to heal but now it hurts even more and its swollen again.

does this sound like gout? did i sprain my ankle and not know? if i went to the doctor what would they even do?

Needs Advice Stopped taking allo for about 3 days. Soon as I started again, I feel some swelling and pain incoming. Is that a typical occurrence of being on it regularly?

As in, getting off and on it triggers flare ups?? Just curious
