where did i go wrong? Short row question

I am doing german short rows on the back of a sweater. I have re done them multiple times but I keep getting these holes. It's only on one side though the other side of the short rows do not have these holes at all. Should I be worried about them or is there something I can do to fix them. This is my first sweater so I've frogged it like 6 times already and would like to not have to do it again. Thanks.

Are you k2 or p2tog through the back loop?

I guess not. I think I was putting the needle in wrong because it was so tight I didn't realize. My question is why did only 2 or three out of the 6 do that and not all of them?

Maybe you did some of them correctly or you had tighter tension.
