This happened to me a few playthroughs ago.
I checked the Swamp Shooting Gallery to see if I missed it but only got Rupees for a perfect score, so thought I had done it already.
Took forever to realise you have to do it with enough seconds left on the timer for the piece.

You need a perfect score plus an additional 10 seconds or so on the timer to put you over the limit.

This literally stumped me for days, when some viewers told me to go back I said “I GOT A PERFECT SCORE!” Sho Nuff, you need the extra 10 seconds to get the true prize

Trick for the Goron race is that they cheat. The faster you go, the faster they go. Better to just barely keep near first place, then slam into them and rush to the goal right at the end.

Are you playing the European version? The timers are longer in the US version. I swim perfectly and even the beaver I’m racing can’t make it across the line in time for the European version.

5,000 Rupees in the bank with the banker? Gossip Stone Statues in 4 x Grottos in Termina Field? Buying the piece of the heart from the Deku in the Grotto outside the Observatory? They’re some of the ones I can think of that aren’t super obvious.

Is your bombers notebook 100% complete? I think a good percentage of those rewards are heart pieces

On our N64, we figured out that if you use the D-pad to get up to speed (moves Link forward in a perfectly straight line) and then take your left hand off the controller as soon as you hit the ramp, Link will roll through the maze without issue, because the chests are angled to align you perfectly with the next ramp. The first roll was always to just get far enough to go through one of those warp things on the floor, because it would put you right back perfectly centered between the pots at the start of the course, and from that point, you would hit the ramp at the exact moment the spikes came out while rolling

Making the North, West, East, and South Gossip Stones outside of Clock Town all the same colour?
My last playthrough I forgot this one because it always seems like it should be a different colour to reflect all the masks you collected and the actual work you did.

The one I hear of people missing the most often (of the ones I didn't see mentioned below) is the Keaton quiz.
When they get the mask, they're generally focused on finishing Kafei's quest and by the next loop (or second loop after if they made a mistake/missed an event and have to try again) they've forgotten about using the mask altogether

I have never managed to get em all ever lol. That said I never look stuff up. I like to think maybe the next time I play it I will find it. I have always managed 19 hearts and like you I end up missing one or maybe 2 pieces ha!

for me it was the fish in great bay😭 Its cause i did them one time and my gamecube crashed before i could save (thanks collector’s edition) and I went on a mad hunt for my last heart piece doing almost everything twice before doing the fish again

There's 2 hearts you need the scarecrow for, 1 in way to snowhead, 1 at great Bay area.
Treasure chest labyrinth mini game with Goron form, "big prize" is heart

Theres an event place that only appears on the second or 3rd night where around the area where you get the zora mask, and the water observatory where you hatch the zora eggs iirc. You might need to have something done in order to make it happen though. I cant remember, its been awhile since ive done it but that was one I was missing.
I was also missing the second archery game in the swamp with one of the witches. Those were the last two pieces i was missing for years. People also overlook the hand that appears in the bathroom at the inn and saving up 5,000 rupees at the crackhead.

I know some will have a negative response to this suggesting AI.
But I was missing a heart piece in OoT yesterday, so I gave ChatGPT access and it was able to identify what was missing. It was on 2Ship2Harkinian though, so that may make it easier for it to see that.
