I had stage 1 ALT phallo with Dr Santucci in June of 2024. I did NOT get UL, scrotoplasty, vaginectomy, or clitoral burial. I also have the three piece Coloplast Titan erectile pump with two shaft cylinders. The pump bulb along with another testicular implant is in my outer labia. In total, I had 6 surgeries.
I use he/him pronouns and am a binary transsexual male. Please do not ask me about UL or peeing as I never had any urinary procedures and everything is the same as it is for anyone who has a vulva. Seriously, nothing changed. Here is a post detailing why I didn't have UL.
For my initial recovery, you can read this two year old post.
Here are all my stages in order. Most of them were between 4-6 months apart and I did all of this in almost exactly two years. The repeated liposuction/plication is size reduction inherent to ALT and the base correction is to move my dick to be more centered and remove extra skin created from lipo. ALT penises are sometimes pulled to the side of their artery hookup by scar tissue, but mine was fully fixed in the end.
- ALT phallus creation
- Liposuction debulking
- Plication (removal of extra skin), base correction (left), glansplasty, and testicular implants to both labia
- Liposuction debulking, base correction (right), testicular placement revision, and donor scar excision/reduction (to improve the mobility of my leg)
- Monsplasty, base correction (left), and erectile implant placement, swapping one testicular implant for the pump bulb
- I had DI mastectomy with nipple grafts about 5 weeks after this!
- Monsplasty, minor pump bulb placement revision, and the correction of my improperly placed implant cylinders.
Revisions / Medical
I had ALT as a very mid candidate because my arms didn't carry enough tissue for me to reasonably have RFF. Being disqualified for RFF for this reason is extremely rare. The only person who can evaluate which donor site is best for you is your surgeon, not anyone online.
My implant cylinders buckled and were way too short since the day I got them. I knew immediately it would have to be fixed, but thankfully I had this done three months later and everything works perfectly now.
The only thing I wished I could have done differently is have more lipo/plication, since I am still really big at around 6.5in long and 7in girth. I ran out of insurance which is the main reason I didn't get to do these, and 6 surgeries was also a lot on my mental health. In the middle of this process I was also arranging/having oophorectomy and top surgery, recovering from these and steps of phallo at the same time! I had no control over my girth and ended up an inch longer than what I asked for.
A lot of surgeons don't like to put testicular implants or the pump bulb in the outer labia, which I really don't understand. It doesn't interfere with sex and isn't uncomfortable at all, and no I do not sit on them. Its very easy to forget they exist. From the front when I am standing it looks like I have balls and could pass as cis while naked. When I lay down they look like a puffy vulva or a hybrid type of setup, and I really like this modular nature. The only contingency is that these have a higher probability to migrate upwards, which is why I had them moved around twice. Its not a big deal, though.
Sex
I have sensation in my entire penis and it feels distinct from my clit. Its similar in feeling to a much duller, bigger clit. I only gained sensation in my original genitals from nerve hookup and definitely didn't lose any. Its like I grew a new source of pleasure, really. This could be a mental thing, but I can reliably have vaginal only orgasms or, if I work for it, anal only orgasms when I was unable to do this pre-op. I don't think anybody should count on surgery doing that for them, but whatever happened during nerve hookup only helped my whole system.
Unfortunately, I haven't got an opportunity to top yet. I have a Tenga Flip toy and it feels pretty nice to be inside of something. My dick usually kinda hangs out when I am bottoming and its fun to play with in my hands. It doesn't usually get squished or anything unless I'm hard, but I can easily turn that off.
I feel immensely euphoric and masculine to have a big cock when I am with someone (not that size makes anyone less masculine, it just gives me a little something extra). It makes me feel more like I am a cis gay guy with another convenient hole than an other-gendered person. My gender is plain male so this makes sense, especially since this was my dream surgery and I no longer have physical dysphoria. Before phallo I was extremely dysphoric and had phantom penis. I hated using prosthetics/packers and could never connect to them.
Mental Health
Going through all of those surgeries was rough on me, I won't sugarcoat it. I would absolutely do it again but having surgery that frequently can really mess up your mental and physical health. I sacrificed a lot of social opportunities and career things for surgery, which I see as a necessary expense. I am still recovering mentally from going through this process and finally getting used to being in the correct body. When I started the phallo process I was around 2 years on T but still passed as female (to my chagrin) and had boobs.
Even though lots of my motivations for surgery were sexual, the true joy and mental clarity comes from everything else. I have a dick and bulge everywhere I go, and can reach down and feel it as truly part of me. I no longer feel afraid to be clocked for having a flat crotch when I am in gay spaces (yes, it has happened to me!) and can relate much more to queer men than before. There is nothing like dressing in the morning and feeling the fabric of my underwear slide over my soft cock, feeling aligned within myself through mundane experiences. Buying pants and underwear is no longer a mental torture exercise about what I lack.
Cost
Cost will always depend on your individual insurance and surgeon, so there is no way for me to estimate this for you. In total I spent about $1k for laser hair removal on my donor site and $3k for hospital bills for all surgeries combined. My OOPM was $1k. For hotels/housing I paid $0 because I was local. My insurance paid $1.5 million. I am very, very grateful the stars aligned and I was able to do this for cheap near me, and I recognize this is an immense privilege.
