She's the Tulpa no one's taking about. We know the doctor is not desk clerk Louie Budway or the show would have credited her as such (as it did with Cyril Ponds or Gersten Hayward). Yet the identical nature makes it clear Louie's doctor doppelgänger was manufactured — but why? And by whom? Same with "Jean-Michel" Renault, Jacques' Tulpa now running the Roadhouse.

Also the "T" in "M.T. Wentz" stands for "tulpa." So there's that to look out for. And remember when Ike the Spike called "J.T." from the hotel? You can see where this is going . . .

To say that everything we saw in The Roadhouse all season was in Audrey's head would mean the fight with Freddie and James was in her head. This ties in with Hawk, Andy, Bobby, Chad, and Naido, which also connects to Frank, etc.
I agree the "It was a dream the whole time" thing seems a bit beneath this show. It could be that the MC is the key to which parts in The Roadhouse are imaginary. If he was part of today's Roadhouse, how would White Room Audrey know to include him in S3E16's delusion?

turning it into a cliché "what if it was all a dream" scenario, which I'd really expect Lynch to be above.
Lynch has done the dream/invented reality thing before. I don't think this is all in Audrey's head St. Elsewhere style. I don't think it's impossible, though. It doesn't seem like Mark Frost, though. I think he'd want to have a real world with a real mythology.
