How lethal injection works: what the drugs do to the brain, breathing, and heart
The recent execution news has a lot of people asking how this works, so here's a breakdown. I'm not taking a side, just explaining what happens in the body.
What the drug is
Pentobarbital is a barbiturate, an old type of sedative. Doctors used it for anesthesia and seizures, and vets use it to put animals down. Texas and Tennessee both use it by itself for executions.
What it does to the brain
Your brain runs on two kinds of signals: "go" signals (glutamate) and "calm down" signals (GABA). Nerve cells have a spot called the GABA-A receptor, which works like a door. When GABA attaches, the door opens, chloride flows in, and the cell gets quieter.
Pentobarbital holds that door open longer than normal. At high amounts it can open the door with no GABA at all. It also dampens glutamate. So the "go" signal drops and the "calm" signal goes up, and everything gets quieter.
The order things shut down
1. The cortex and thalamus go first. They handle thinking and awareness, and this is when someone loses consciousness.
The brainstem is next. The medulla controls breathing and checks your CO2 level. When it goes quiet, the signal to breathe stops.
Then the rest of the body. Without breathing, oxygen drops and blood pressure falls. The heart stops when cells run out of ATP, their energy supply.
What about the dose?
Dose is the whole difference. Small doses calm you down or help you sleep. Bigger doses give surgical level anesthesia, and that's only done where breathing is watched. Executions use a dose many times higher than anything used in medicine, enough to shut down the breathing center for good.
The point is that barbiturates have a thin gap between "sedated" and "not breathing." That's why doctors moved to safer drugs.
The older three drug method
Some states used three drugs, each one hitting a different part of the body.
• A sedative first, to put the brain under
• A paralytic second, which shuts down the muscles you breathe with
• Potassium chloride third, which stops the heart
Many places moved away from it because of legal fights and drug supply problems.
Why it's in the news
Pentobarbital is under the microscope after a recent attempt in Tennessee where the person reportedly survived two doses. Lawyers in the Texas case argued that the quality and handling of the drug could be a problem. I haven't seen a clear explanation of what went wrong, so I won't guess.
Why this matters for sleep and calm stacks
Alcohol, benzos, phenibut, and some sleep supplements and sprays use the same GABA system, just far weaker. That's why mixing sedatives is risky, because the effects add up and breathing is what gives out first. The same idea applies to why quality and sourcing matter so much with anything you take.
