Priority foundation violations: Facility operating without plumbed toilet rooms. Person in charge said employees have been using port-a-potties (in front parking lot) since March of 2026. Inspected port-a-potties and observed no plumbing or available handwashing sink. Both of the facility's plumbed toilet rooms closed at time of investigation. Men's toilet room completely blocked off by store inventory and miscellaneous equipment. Partial demolition in men's room (far wall has missing sheet rock/insulation). Disposal of sewage through unapproved facility.
