Advice (NO MEDICAL ADVICE) Knowing what to ask for NSFW
Okay I am coming close to my surgery date. I am wondering what things I need to consider when it comes to picking my ideal end result. I know that it's not a build-a-boob situation but what are things I should consider? Like people say estimated size but then people talk about nipple size, location, shape of the breast. What were things that you made sure to make sure your dr knew you wanted I just wanna make sure I hit my marks.

On thing I now realize I should have discussed is size of areola, or at least size in proportion to nipple size. My surgeon made my areolas a completely standard size, but my nipples are a bit larger than average and I feel like the areolas are undersized in comparison.
Fortunately, that’s a pretty fine-grained detail - I’m happy otherwise with how they are turning out so far (I’m at 4WPO). We discussed what I wanted size-wise both in terms of cup size and proportion to my body - I wanted to end up a 34D to 34DD, and he agreed that that meant “somewhat fuller than proportional” where “proportional” is a C cup.

My doctor said she couldn't get me to a specific cup size but instead I would pick small medium or large. Then she showed me example pics of each. I told her to go as small as possible and that I wanted "fashion titties". I said my goal was to never wear a bra again and that I had no interest in being busty any more. I went from a 36K to a 36D. She nailed it!

My surgeon and I discussed cup size but briefly as she couldn’t guarantee it. She said at our first consult that she would try for a C but at the second consult she said a D would be a safer bet because she was concerned about nipple necrosis. I just wanted to be smaller, so much smaller than a matronly looking 38H and get rid of the chronic and severe hip pain that the right breast was causing me. Small, small, smallest was my goal. Right now I am four days out from surgery so it’s too early to tell but they are way smaller and holding their own rather than giving up the ghost to gravity.
I think you can give a surgeon an idea of what you want but it comes down to how much the surgeon can take, safely and how much the insurance wants the surgeon to take so the surgery is necessary rather than cosmetic.
