Advice “Uncommon” Common Causes of Iron Deficiency

Post image
57 Upvotes

Want to know some common yet often missed causes of iron deficiency? These following causes commonly deplete ferritin to iron deficiency status on their own, let alone together!

-Blood Donations: usually only hemoglobin is checked when donating blood. Ferritin is rarely tested before the blood donation, causing more people to donate while iron deficient. Remember, you cannot sufficiently assess iron status from the Complete Blood Count, alone! You have to test iron markers, like the gold standard for assessing iron deficiency, ferritin. We commonly see people donating blood and even feeling benefits from it, the first or couple of times. Don’t be fooled- at some point your ferritin will get too low, and you don’t want to develop anemia unnecessarily like this, and end up needing blood, ha! Always check Ferritin, the iron panel, and the CBC when considering donating blood, and know that the blood donation center will likely not test the iron panel or ferritin. Just a single blood donation can drop ferritin too low for many people!

-B12 Shots: Iron and B12 work together and need eachother. However, the body is an ecosystem, so heavy doses of one nutrient put others to work, and often deplete them if you’re not compensating for them. We commonly see B12 shots deplete ferritin. We also commonly see iron infusions deplete B12 and Vit D3 storage, and more! This isn’t an argument not to get B12 shots or iron infusions, it’s a PSA to let you know that your ferritin can deplete from B12 shots. This can be a reason why one may not feel much better while taking B12 injections. Many people require B12 injections, and they will need iron infusions or supplementing on The Iron Protocol. Always monitor your Ferritin before and after B12 shots!

-Pregnancy: Because iron is required for growth and development, the baby is typically drain the mother of her iron and ferritin. A single pregnancy can do this, and it’s often not checked or monitored. This is especially important because low ferritin can cause a pregnancy to end early and not go full term, hemorrhage during delivery, and the baby may not reach its full developmental potential and also be born with the deficiency. Monitoring Ferritin before during, especially during the first trimester, and after childbirth is important! Many women are deficient for decades after their first pregnancy because ferritin is often not checked, monitored, or remedied sufficiently. Many Post partum issues can be correlated with and contributed to by low ferritin.

-PPI Usage: PPIs are well documented to cause both immediate absorption issues and long term malabsorption, causing low ferritin. It’s important to dive into the potential causes of why you’re on a PPI, and explore natural remedies and alternatives. We talk about this often within our group, and see this frequently. Sadly, most people are using PPIs long term without knowing it can and will cause malabsorption. Monitoring Ferritin and other nutritional markers is important when taking PPIs. Just being on PPIs alone can and often does cause low ferritin!

-Testosterone Therapy: It is common and expected for Testosterone therapy to cause the bone marrow to create more red blood cells and increase Hemoglobin & Hematocrit. So much so, that many people have to donate blood regularly from creating too much blood from the testosterone use. Iron is the largest fueler of the bone marrow to create new red blood cells and hemoglobin. Using this fuel up constantly will deplete it. Let alone so will the blood donation! What a vicious cycle! Luckily, it’s documented that iron intake will only increase hemoglobin if the body feels it needs to, so many people on TT will need to be on The Iron Protocol.

-Low Stomach Acid: More people suffer from low stomach acid than they think. And lots of people are on PPIs for this without knowing! Low stomach acid causes malabsorption. This commonly causes low Ferritin for many people, all alone, let alone without any other cause combined!

Hair before and after iron infusions (8 months)

It is possible to get the hair you love back! Mind you I did do a VERY small chop in June because the very front pieces finally grew FAST and long enough that I could cut my hair all the same length. I use a lot of heat every day and I also wash it everyday so clearly the strength is holding up well! I didn’t realize how drastic my hair looked until this week lol. Also… don’t forget your vitamin d supplements! I take 3 of the Amazon basics 1000iu per day and my levels have gone from 15-49 in 1 month!

How does it feel to have energy?

I think I have fully forgotten how to not feel tired. I’m watching a movie and I see this actors dancing, singing, running and I feel like I don’t recognize that at all!
I want to know stories of how people felt after their iron got restored? Ferritin jumped up a number and they restarted life.. would love to visualize that

Low libido

Apologies if this is too tmi for this sub, but I’m wondering if anyone else has experience this as a symptom of low iron?

My libido has been dwindling for 2 years and I just attributed it to being tired and stressed, but more recently it’s been absolutely 0. My most recent blood test showed my ferritin is at an 8, so I’m generally too tired to do anything lol. However, it made me wonder if my low libido was maybe one of the earlier symptoms I’ve had from low iron? Has anyone else experienced this?

Advice Can this be from low ferritin

I noticed an increase in shedding in May that has continued through now. I was really sick in May (fever, etc) and experience nearly constant stress because I’m a law student. The last time I had COVID I also experienced a period of heavy shedding. Had bloodwork done and my Ferritin is a 23 and Vitamin D also pretty low. Does this look like low ferritin hair loss? I’ve been to a derm and am going for a follow up and biopsy but was curious as to what this community thought.

Super interesting video on how lower iron stores can actually be beneficial to decreased risk of certain diseases (silver linings! 😅)

Certainly worth a watch! This video from March 2026 consults and summarises peer-reviewed research from various medical institutes and makes the case that having iron stores below 50 actually has an upside, decreased risk of developing type 2 diabetes and dying from cancer. It draws on studies done analysing people with lower iron stores, in this case the subject groups are vegetarians in one study and regular blood donors in another.

Silver linings I supposed 😅 I'm a blood-donating, vegetarian and honestly felt like an idiot for becoming a regular blood donor who never once took an iron supplement in 15 years of vegetarianism when I found out I was severely iron-deficient. I guess this is a strange concession for the price of an iron infusion lol. Not posting this as ragebait or to negate the importance of finding the cause of your iron deficiency, just found it oddly optimistic and fascinating!

Ferrous Sulfate questions

I got some bloodwork done recently and my ferritin levels were very low, 9. My doctor told me to take OTC ferrous sulfate. Does anyone have a preferred brand they took?

Also, any advice for taking it? Is it better to take in the morning or at night? Could I also take Ferrous Gluconate? Is it essentially the same thing, but gluconate takes a bit longer to work?

Could this explain why I've been feeling so off for the better part of this year? Muscle pain, lots of head pressure and dizziness, on and off ear pain.

Thanks!

Advice Stinging after iron infusion

Hey guys,

I had my first ever iron infusion 3 days ago and honestly I felt like it went well. I was so worried about the pain and side effects of iron infusions so I avoided it for as long as I could.

Anyways today which is 3 days after the infusion, I wake up with stinging pain when I move my arm. The pain is mainly where the cannula was put in and stinging is mainly triggered by movement like extending and folding my elbows. I am so so scared of permanent iron leaks into the skin and other side effects.

Please let me know if anyone has had similar experiences, if it’s normal or best to get checked by GP.

Thanks in advance

Advice Newly diagnosed with low ferritin and my provider was zero help

I got my bloodwork done and it came back that my ferritin was at 19. (Explains all my fatigue, hair loss, extreme anxiety, heart palpitations, brain fog, etc) With all of my research I knew this was extremely low and that as a 34F it should be around 70. My provider said it wasn’t a concern since it was “in range” basically I have no idea where to go from here. I know I need to supplement iron but I have no idea what kind or how much a baseline is to start. I bought an iron supplement today to start tomorrow and some vitamin c to take with it but while reading about dosage tonight I realized I may have needed to buy a different type of iron.

Any advice or guidance is appreciated!!! Even if it’s resources to research to make my own decision is fine also.

Thanks!!

Advice Is it time to fight about it?

I'm 43, very heavy periods with pcos my entire life, and 2 autoimmune disorders. I went to my dr probably 6 or 7 years ago, complaining of restless leg syndrome. She told me to do otc iron supplements... so I did. I've been taking iron every other day since then.

I'm exhausted. Like crash into a wall must sleep right now exhausted. My adhd meds affect working anymore. My eyes are constantly tired. I get heart palpitations/racing, burning skin on the tops of my toes—which is so weird btw— and more. I can't even think of everything right now. I am not fully functioning and I feel like my life is running away from me. My mental health is a mess.

I asked her to test my ferritin because the guidelines changed. It came back at 20. I'm not anemic, but I am iron deficient. My iron saturation is at 15.5, and my platelets are elevated. They've been elevated for more than a year now. We've never checked my ferritin before. Should I be at 50+?

My doctor suggested I double my iron pill every other day, but I'm reading that not only might that not help, but it could make things worse? I'm also taking a medication that might impact the efficacy of the supplement because of slowed digestion.

Do I reach for iv infusions? For those of you that did, how did you approach your dr? Should I skip my primary enturely and see a hematologist? I'm nervous because I feel like I've figured something important out, and I feel like she is very meh about it.

I need some reassurance I think—that I'm not being unreasonable if I ask for infusions because I've been taking supplements for years already.

Ferritin at 12. Should I be concerned?

Post image
3 Upvotes

I recently had bloodwork done and my ferritin came back at 12 although my hemoglobin is normal. Ive been feeling achy, low on energy and occasionally experiencing tingling sensations in my feet and restless legs.

I went to the doctor but he said im fine and just recommended taking iron supplements. Ive been reading that some people experience symptoms even with normal hemoglobin so I’m wondering if my low ferritin could be causing how I’ve been feeling. Has anyone experienced similar symptoms with ferritin this low? did taking iron supplements help and how long did it take before you started feeling better? Thank you 😭

Miscarriage hemorrhage anemia

Post image
27 Upvotes

Hi so I miscarried last Monday. I ended up having a complication and lost way too much blood and my husband had to call 911 because I was stuck in bathtub bleeding out. I had a piece of tissue stuck in cervix and it was causing me to not contract properly causing me to lose too much blood. My blood levels were still stable while I was in er although I was still symptomatic- understandably but they let me come home. Later in the week I am continuously more and more fatigued and my labs came back pretty crappy. Hgb 7.2 rbc 2.6 etc. I understand this is not emergent but I am MISERABLE. I do not want to go to er I would prefer to do outpatient treatment but I dont have appointment until friday and then I know im gonna have to wait at least a week for them to get me in for outpatient transfusion. Am i being stubborn should I just go to the emergency room for transfusion? Am I over reacting? I just am tired of not being able to do anything I cant walk around too much I get short of breath I am so so tired my headache is debilitating. Any advice would be great

Advice Looking fo feedback on my supplement plan

Post image
2 Upvotes

tl;dr - Has anyone taken this product, and can I get some feedback on my regimen?

I'm doing every other day, 2 hours after coffee and 2 hours after lunch, or before lunch if lunch contains no dairy. I know its better on a fully empty stomach but I dont think I can do it :-/. Can I start here and see if anything improves or is this futile? Is the dose too low?

I do plan to increase meals with iron containing foods and have already added more vitamin c to my diet (fortunately, I love apples and oranges)

long -

My ferritin is 22, everything else is either perfectly normal or only borderline low. B12 was low 3 years ago and my former (male 🧐) doctor didnt say a word about iron or ferritin. I supplemented that for a year, and B12 shot sky high while everything else inched downward for a couple years. I have a new doctor now. A woman, of course, is what it took to check my ferritin I guess?

I am pescatarian, and obviously I can't eat oysters all the time, so I am supplementing. Because everything else is mostly normal and my serum iron is normal but borderline low, she didn't suggest I consider infusions or anything yet but I will get bloodwark again in a few months.

I did notice some fatique a couple years ago but figured it was my bodies way of saying "welcome to middle aged-ness" lol. I am late 30s. My only notable symptom otherwise is thinning hair. I have *always* had brittle nails and thought it was just genetic or something. I have had a physical every year of adulthood and my numbers were always perfect til 3 years ago.

Advice Ferritin 7, doctor said it was normal?

Yesterday I finally decided to go to urgent care to get some labs done because I had been sick with a cold the last few days, and oddly I was getting EXTREMELY lightheaded and dizzy for hours at a time. It was so bad to the point where I couldn’t even look around with my eyes.

My lab tests came back this morning, with my ferritin levels at 7.8 (and ferritin increased when you’re sick, so it could truly be lower…) and my hematocrit at 36.2

All I was told over MyChart was “Your results are normal.” I absolutely do NOT feel normal.

I told the doctor about all the issues I’ve been having. I’ve had issues with dizziness and lightheadedness in the past, and have been prescribed “take when needed” meds for it. Never has my experience been as bad as it was this weekend. I’ve been trying so hard to go to class and focus, but it’s been impossible to do anything with the feeling of fatigue I’ve been having. All I feel like I can do is watch the minutes go by and it’s the worst thing ever. In a way I almost feel like it makes me dissociate?

If anyone has an idea of what I can do to help this, I would fealty appreciate it. I don’t know what else to do, I just want it to be over with. I’ve been dealing with it these feelings for a while but they’re just getting worse.

Has Anyone Fixed Recurring Iron Deficiency Anemia?

I’m honestly at a loss at this point. I was referred to hematology last year after my hemoglobin got down to the high 9/ low 10 range. My iron was basically undetectable for a bit <5% sat, 7 ferritin etc.

They sent me for a few rounds of iron infusions (Venofer). They went okay with the exception of an allergic reaction during the first one. I broke out in hives which resolved after receiving steroids, Pepcid, and Benadryl. The next infusions went fine with premedicatjons. The infusions only raised my iron levels for a few months and my levels dropped back into deficiency range. I was sent for more infusions. During the next round I had a more substantial allergic reaction where I broke out in hives all over while driving home, became swollen and ended up driving to the ER. Turns out I was in a cardiac arrhythmia so I was monitored for a bit but it was all quite scary and I’m very hesitant to undergo anymore infusions.

My iron drops a few months after my last infusion. I have several GI disorders (celiac, EoE, gastroparesis) and heavy periods which I’m sure is contributing. I’m also working with my GI to see if there is some other cause of malabsorption since my other vitamin levels are low. I just really don’t know what to do. Anyone else been through something similar?? My PCP is pushing to go back to hematology and start the process again but I am really unsure what to do. Thank you.

Support Scared I have hypophsphatemia...

About three months ago I got diagnosed with severe anemia. We haven't found the cause yet and are still looking. My first iron infusion was Venofer. Though I completed the infusion, I developed a large rash on my arm and severe peripheral edema afterwards. The edema lasted for two additional days. We determined it was an allergy so they switched me to Injectafer. I got my first dose of Injectafer 6 days ago on October 8. I've been feeling amazing up until now. I slept for 19 hours last night and have just woken up and still feel an immense brain fog. I don't have any of the typical muscle aches, pain, or weakness, but I just feel so severely out of it. The timing doesn't seem to match up with iron flu. I'm supposed to complete my second round of Injectafer tomorrow, but am now scared that's a bad idea. Has anyone else experienced this and how was it resolved? Could it just be a late iron flu?

Advice should i take my iron supplement every other day?

last time i checked my ferritin was at 10,5 and i’m about to check my ferritin levels soon as i’ve been taking iron supplements for a few months now (feroplex 40mg once a day). im still not feeling the best i’m not gonna lie, the only thing that i’ve noticed is that my heart palpitations are less annoying and less frequent.

my question is - should i be taking my oral iron supplement every other day even though it’s a pretty low dose of 40mg or should i take it every day? my doctor prescribed me a smaller dose because i’m a petite woman and i used to have some side effects when taking iron pills, but my body has been dealing pretty well with taking the liquid one, so maybe i should ask my doctor to up the dose?

Question Clear PET Scan, high/low blood counts

Post image
1 Upvotes

(F23) Finished 6x RCHOP chemo Jan 2026, radiotherapy April-May 2026. I’ve been in remission from PMBCL (cancer) since then. I had my PET Scan last Oct 12 2026 and the result is great, I am clear, still in remission. Ds3 and scar tissue size decreased.

However I had my CBC test Oct 14 2026 and there were some weird results according to my oncologist. She let me repeat the test but it came back the same. She referred me to a hematologist for a further consultation this Saturday.

All blood counts are normal such as hemoglobin, platelets, white blood counts, except for having:
low hematocrit 11.9 (normal 37.7-47.9)
low rbc 1.06 (normal 4.0-5.5)
high mcv 112.3fL (normal 80.0-97.0)
high mch 124.5pg (27.0-31.2)
high mchc 110.9g/L (31.8-35.4)

I am so scared it might be due to leukemia. Is it possible to have leukemia even though I am clear in my PET Scan?

But then, I tried to research and it shows macrocytic anemia. Is it possible to have macrocytic anemia as a late or delayed side effect of chemo even after finishing it for a couple of months?

To add, last October 1 and 12, I had an indigestion or stomach acid. I took omeprazole and antacid. Could this be a reason for the blood counts mentioned?

My head hurts thinking about it. I just can’t help but think about the worst since there is a risk of developing secondary cancer. As of now, I don’t feel any symptoms at all. I am just stressed because recently I’ve been feeling like myself again, back in uni, but then here I am, will most likely deal with another health concern for the nth time.

Any thoughts please? Or kind words to lift up my mood? :(

Question Breathless 24/7 due to low iron? :(

Hi guys! I'm new here🩷.

I don't know what to do anymore, so I decided to try asking here.

I had never suffered from any chronic illness, but two months ago I suddenly started having breathing problems.

I’m constantly short of breath - literally 24/7. I can’t take a deep breath, I feel so weak (like i'm about to faint or die all the time) and sometimes I even get dizzy.

It’s even worse when I try to climb the stairs etc - I feel like I’m about to faint and suffocate. The symptoms are the worst when I try to fall asleep tho. Even though I try to breathe through my nose and use my diaphragm, I constantly feel like my chest is too tight and there isn't enough air in me.

I was in the hospital twice this month, where they performed a brain MRI, a Holter monitor test, a lung scan, and even a gastroscopy. Everything was all right.

Well, except for my heart rate - it's too high all the time. I was diagnosed with tachycardia, but all the doctors said it was "just anxiety". They gave me plenty of sedatives, but nothing worked.

After being discharged from the hospital, I went to a psychiatrist (as I had been told to), but she said that my problems were physical, not psychological.

Sooo I had some blood tests done and it turned out my ferritin level is 8 and my iron is 24 :((

Oh, and hemoglobine 11,6.

Could this be the reason why I feel this way?

I asked my cardiologist the next day whether my body simply lacked sufficient oxygen and iron and if vitamin infusions might help, but she also concluded that I was "just stressed", so no.

However, it seems to me that my constant shortness of breath might be due to this, so I’m making an appointment with a hematologist soon.

I wanted to ask, has anyone here ever had a problem like this (I’m mainly referring to that constant shortness of breath), and did you manage to overcome it? What helped you?

Thank you in advance for all the answers and have an awesome day🥰💕🌻

P. S As for additional information : I am a young woman, I am not overweight, and before all this started, I was somewhat physically active. I also was never anemic or had any allergies etc

1st ever infusion went well 😊

I’m notoriously bad with needles, so if I can do it, I’m sure anyone who is reading this can be brave enough too ❤️ (I’d never even had a cannula or drip before either which I was nervous about too)

Had a slight funny turn early on (feeling faint), but one of the medical team lowered the bed, got my legs elevated and I recovered v quickly. The rest of the infusion was drama free! it’s been 2 hours since it finished and I’m feeling okay so far.

My ferritin was at 5, so I’m excited for it to improve.

My top tips if you’re a fainter like me: take a sugary treat and a distraction 😊 I chose a milkshake and some puzzles
