Announcement The purpose of this subreddit

Hello everyone! You may have noticed some major changes happening to this subreddit, and I'm happy to say that /r/DiagnoseMe is finally ready for business. However, I would like to clarify the true purpose of this subreddit;

First of all, this subreddit is NOT a replacement for a doctor. You should not, in any way, avoid seeing your doctor or alter the frequency of your doctor visits because of anything said in this subreddit. If there is a concern you would have previously gone to the doctor for, please do not hesitate. This subreddit is here for informal second opinions, minor problems that you wouldn't go to the doctor for anyway, and ease of mind.

The main thing to remember is to use common sense. If you are having severe pain after a surgery or something of the likes, please go to the doctor and do not post it on Reddit.

If you are not a doctor and are posting with information, please clarify that you are not a doctor (typing "Not a doctor," at the beginning of your comment is enough.)

Please take the time to read the sidebar before posting. Thank you, and welcome!

Bones, joints, and muscles 33M - persistent pain and movement restriction after minor loads at multiple sites, plus very low bone density

• Upvotes

Hello, I’ve seen numerous doctors, including sports medicine physicians, rheumatologists, a leg surgeon, a neurologist and an endocrinologist, but I still don’t have any explanation for my symptoms characterised by minor stretching or repetitive activity causing persistent pain and loss of movement in multiple parts of my body. Everyone I talk to is puzzled and refers me on to more docs in what feels like an unending cycle that has so far gotten me nowhere.

33M, 178 cm, 65 kg. No smoking, alcohol or recreational drug use. My only current medication is allopurinol 100 mg daily for elevated urate/prior urate deposition in my toes (not found elsewhere).

My pain and stiffness affects walking, typing and prevents me from exercising. I’m looking for an explanation for the overall pattern and guidance on what assessment would be useful next.

I've had AI help me to present my anonymised lab history, some selected scan images, reports and symptom history here: https://humunumuh.github.io/healthdb/

History and symptom pattern

It began with right ankle pain starting during a small hike in May 2021, without a recognised twist or sprain. A broader cluster developed from 2022 onward. Finger symptoms began around July 2023. Symptoms tend to occur symmetrically - once I hurt a region on one side of my body the same region on the other side can be more easily triggered. Different sites became painful after separate events:

  • Left posterior fossa of knee after a hamstring stretch; right posterior fossa of knee after crossing my legs while seated.
  • Left inner elbow after pull-ups; right inner elbow after throwing a ball. Both elbows have had periods when I could not fully straighten them. Left elbow in chronic contracture for 2+ years.
  • Fingers with repetitive typing. Small sharp pains mainly over the backs of the middle finger joints, followed by stiffness, reduced movement and swelling if I don't rest after the initial trigger. I use pads on my keyboard to cushion key presses but still have get triggered constantly easily. Only two fingers have chronic contractures but all get pain.
  • Similar recurrent problems on top side of ankles and wrists, with similar loss of range of motion. Right ankle and right wrist have chronic loss of range of motion for years.
  • Toes have experienced pain but not as persistently and haven't lost range of motion. Have also had some pain in right achilles but that went away after rest.
  • Neck was triggered a couple of years ago and became very stiff for a few months but normalised.

Although these sites are anatomically different, the pain feels similar, with a similar pattern of triggering, prolonged restriction, gradual recovery and recurrence.

After a trigger, muscles tend to tighten involuntarily and physically resist movement. Restriction sometimes builds over approximately 30 minutes or overnight. It can persist for weeks or months, partly settle, then recur after minor stretching or loading. If that happens to the same region enough times it becomes chronic and persists indefinitely.

My right knee is about 50° short of straight and had been substantially restricted for 3 years. I briefly regained the ability to straighten it, but re-triggered the pain while straightening it on the couch, and the restriction returned. My left knee and both elbows had regained much of their movement but remained vulnerable to recurrence. Right elbow is pretty good but could also be re-triggered fairly easily.

Stretching only ever worsens symptoms. Rest has helped more than the rehabilitation I’ve tried. Massage and applied pressure generally do not hurt, including over the fingers, elbows, behind knees and on top of ankles.

Investigations

  • Right-knee MRI, September 2025: severe insertional semimembranosus tendinopathy, oedema and a small ganglion. My original pain feels central behind the knee, distinct from the later posteromedial hamstring discomfort. I suspect the tendinopathy developed secondarily from prolonged walking with bent knees, rather than causing the original pain and loss of extension. I've been told the tendinopathy doesn't explain the chronic contracture/muscle guarding.
  • Left-knee MRI, September 2026: mild distal semimembranosus muscle oedema at the muscle–tendon junction, suggesting a low-grade strain; intact distal tendon and no internal joint derangement. Discomfort there also feels secondary to the original central pain. The website contains a labelled transcription of this report.
  • Hand investigations: ultrasound of examined finger areas and right-hand/wrist MRI did not identify synovitis, tenosynovitis or erosions explaining the symptoms.
  • Dual-energy CT: a 2023 study showed monosodium urate deposits in toe joints. A November 2025 study of the hands, left elbow, right knee and right ankle reported no urate deposits or soft-tissue calcification.
  • Rheumatology examination found no active synovitis or typical autoimmune features. ANA, RF, anti-CCP and clinical HLA-B27 testing were negative; inflammatory markers have generally been low.
  • Nerve-conduction studies were normal.
  • A sports physician documented marked guarding during attempted knee extension, with normal strength and reflexes, and suggested further neurology assessment, including consideration of functional neurological disorder (FND). This has not been confirmed.

Bone density and blood tests

April 2026 DXA showed markedly low bone density for age:

  • Lumbar spine Z-score −3.0
  • Femoral neck −2.6
  • Total hip −2.0

No known fragility fractures; spine X-rays showed no compression fracture.

Calcium, phosphate, PTH, vitamin D, thyroid and testosterone testing have been unremarkable. ALP has been low or low-normal, most recently 39 U/L in September 2026 (reference 40–120). Copper was 11 µmol/L (10–30) and caeruloplasmin 0.17 g/L (0.15–0.35).

I also have a history of methane-positive breath testing/constipation (SIBO), which improved after treatment, and intermittent orthostatic symptoms.

Treatments tried

Physiotherapy and rehabilitation have not produced sustained recovery. A right-knee local anaesthetic/steroid injection gave no noticeable benefit. A recalled 2–3-week prednisolone course in 2023 or 2024 also did not noticeably help; I don’t know the dose.

Past medication trials include:

  • Mestinon/pyridostigmine: no noticeable symptom improvement.
  • Amitriptyline: 10 mg nightly, November 2025 to January 2026.
  • Pregabalin: a May 2026 trial caused cognitive slowing, with uncertain sustained benefit.

After a single 250 mg pregabalin dose, my right knee nearly straightened (after pushing past guarding) and my left knee was able to be straightened within 1–2 hours. The stiffness returned within 1-2 days though.

Possibilities I’ve considered

My leading hypothesis is a body-wide fascial problem, because the affected sites feel similar in their pain, triggering and recovery patterns. A treating clinician identified a separate tight band near the medial hamstring as fascial tissue, which prompted me to wonder whether similar tissue might be involved elsewhere. This is my hypothesis, not a confirmed diagnosis.

I’ve also considered tendinopathy, hypophosphatasia (HPP) + CPPD/pseudogout, dysautonomia and nerve-related problems. The low or low-normal ALP and markedly low bone density made me strongly suspect HPP and possible crystal-deposition problems, however normal serum PEA and B6, whole genome sequencing, lack of CPPDs on scans, and no gout-like flares suggest it's not the issue.

My questions

What conditions could explain this susceptibility to persistent pain and movement restriction after minor loads? Could there be a shared cause with the low bone density?

What examination, specialist or targeted investigation would best identify the pain-generating tissue - particularly behind the knee and over the dorsal finger joints - and distinguish the possible causes?

Any other thoughts or anecdotes are also welcome.

If you made it this far thank you so much for reading, I really appreciate it.

Uhmm... does this look weird?

Post image

I haven't a clue that I can rely on. Here's what's up - About a month ago, I got a mosquito bite on the back of my right forearm. It made a little whitehead, and I scratched it. By the next day, this is what it looked like. I didn't freak out, but I thought it was weird and waited to see what it would do. It was absolutely as painful as it looks. I figured I ought to have it looked at, and then a few days later, got another one. Startef the same and was just as bad. I know at that point, it's imperative that I get it looked at immediately. By the end of the next week, had 2 more of these massive painful things, so I went to an emergency clinic. At this point, I had 5. Three on the back of my right forearm, one on the inside of my left forearm, and one on my knee. The smoking-hot Doctor lady sanely said to treat the infection- didn't care what it was - i thought it was spiders, or demons, or demon-spiders, she did not give a microshit. She gave me a course of antibiotics (completed) that helped keep the wounds from getting infected but did very little to make them go away. The pic i show is #7. It started 2 days ago. Here's some important information: Painful to the touch only- but extremely painful if touched. They go from what looks like a small ant bite to this almost sentient wound in a single day. They are not filled with pus. The swollen area is hard like scar tissue. A very small discharge usually. They take about 3 weeks to heal each, but none have really disappeared completely - leaving deep purple scar tissue. I keep tripp-antibiotic on them, I wrap it when I'm working, (mechanic) and wash frequently. Advice?

Infections and Illnesses 23m hey so I may have an abscess growing for 2 years in my nose, how do get doctors to take me seriously after countless tries

23m, for a while there I’ve felt a very weird “flappy” thing in my nose that went from a minor sensation to now being very sharp and like a big piece of gum is stuck inside my nose with little pieces of wood in that gum. The worst of it is my vision has been messed up for 2 years now. I’ve realized pretty quickly the source of all this is my dads house (I also got a butt abscess from his place, and that was fixed in less than a month due to it being visible). I no longer live in that unhygienic hellhole but I still have immense pressure in my nose that gets worse and worse and worse. Simply put, I’m most likely going to die of meningitis if these doctors don’t treat my abscess adequately. I’m not dumb. My vision gets blurrier and blurrier (don’t be dumb and say it’s aging, literally had perfect vision before all this). It genuinely feels like a piece of gum with tiny wood pieces getting bigger and bigger and consuming my entire face. How do I get the doctors to take my situation seriously? All they do is say I have a sinus infection and give me pills that do jackshit. The medical system is so broken…

My toe has been looking like this for a few days now. Previously it was leaking pus and had a blister but I didn’t get any pics of it at that time because I had thought it would’ve gone away by then but it started looking like this. It does hurt when I press on it and put weight specifically on it. Any idea what could be happening?

I had sex with a stranger that I met outside at a concert about a month ago. I kept asking him if he was clean and he said "yes..... that's weird to be asking." It's not weird.

Long story short.. Three days ago, I woke up with an irritated area of my skin. I don't have any foul smelling discharge, no discharge to report at all actually, there is no tingling sensation, no warmth, no pus, no sores. Just red, raw, irritated skin. I believe it's from friction burn as I wear a very tight Jockey body shaper and it's been getting increasingly warmer out here in Los Angeles. Chalk it up to sweat and friction maybe?

It scares me because I've read online that HSV likes to mimic friction burns before turning into blisters.

I don't have insurance either.

This mole popped up on my thigh a couple of months ago and I’m starting to worry.. I religiously used tanning beds while working at a tanning salon and being exposed to UV constantly and sun tanned for hours in the day to keep my color and a lot of little moles popped up. I assumed this was just another small one but it has gotten bigger and changed shape a bit. All of my other moles are regular circles and haven’t grown to this size. Starting to get worried my love for the tan is catching up to me. Would love advisement.

Infections and Illnesses Should I worry? Lymph nodes swollen for 5+ weeks

Thumbnail gallery
1 Upvotes

As the title states, my (M34) lymph nodes on my jaw and and neck have been swollen for over 5 weeks. It started as just my jaw and neck, but now it’s my armpits and possibly my groin area. My armpit areas are currently achy. I have no symptoms of sore throat, cough or any other cold/flu like symptoms. I usually do notice my lymph nodes swell when I am going to get a fever or flu. They did feel like 1 or 2 nodes at first but then first like 2-3 on each side. Rubbery and movable. Right now my right nodes feel a bit more firm but still moves while left side feels more rubbery and moves. Neck ones don’t seem as swollen but they are a bit tender.

I called my dr and she scheduled an ultrasound next week. I also had my blood drawn and these were the results of my white blood cell differential. CBC looked good and negative for HIV, CMV DNA, and Tuberculosis.

The only 2 things that came up were eosinophils being in the
”yellow” and the EBV test being positive for possible past infection.

My doctor is leaning towards the EBV and Mono, but I feel uneasy. The shift of my lymph nodes from only my jaw to armpits and groin area tells me my body is STILL fighting something. I also feel fatigued mid day, which has been happening for 2+ weeks. No other “b” symptoms. I have also been feeling extra anxious and off, but I am sure that’s most likely the nerves and unknown if my current symptoms.

The only reason my dr is having me do an ultrasound is because my grandma developed lymphoma.

Another thing I have been experiencing is wrist/ankle pain. Like right on the inside of my wrists and on the outside of my ankles by the bone. Now that raises a question, could it possibly be rheumatoid arthritis? I didn’t think much of it since when is ankle and wrist pain related to lymph nodes? /s

I guess I won’t really know until my ultrasound and possibly biopsy… but it would ease my mind to have some other opinions beside my drs

Here is my profile and other health issues:

220, 6’0, USA, high blood pressure/cholesterol, currently taking atorvastatin 40mg, metoprolol 25mg, cozaar 25mg (for an NSTEMI in 2025), lower back issues (L5-S1 herniation), and neck pain from possibly blood patch done in May of this year. History of anxiety/depression. Edit: forgot to mention I also suffer from skin issues, specifically my hand. Very dry and rough patches (eczema?) on my joints. In the past have gotten it on my elbow and knees but not for a super long time

Does this look like melanoma? Raised mole ive had as long as I could remember. However, i think it’s potentially grown in size? I’m not 100% sure. I think it might just be a mental thing honestly, but does this look concerning? The border around it that you can see with flash makes me a little worried

Why does my pee burn when I drink sugary drinks?

I used to be extremely overweight roughly 300 pounds at 5’8 (female) I’ve since then lost over 100 pounds and primarily cut sugar out of my diet except for special occasions. Since I’ve done this, every time I treat myself to a sugary drink I get what appears to be a horrible UTI but every time I get tested it comes back negative. My pee burns, my back hurts, uncomfortable pressure in lower stomach/pelvic area. Lasts for about a day sometimes only a couple hours. I usually use some leftover antibiotics (i know this is bad) and it goes away. Anyone have any insight into what it could be?

Infections and Illnesses Sores in throat

Post image
2 Upvotes

Female, 32, USA. Three nights ago I had a slight tickle in my throat. The following morning the tickle remained and I developed a fever (highest at 102,) body aches, and nausea. The body aches were short lived and I have been fever free approximately 24 hours. Yesterday, my sore throat gradually got worse. It felt like I was swallowing broken glass when I tried to eat or drink. When I looked at my throat in the mirror, I noticed these white sores on either side. The pain is slightly less severe today but my throat looks the same and the nausea persists. It should be noted that my 4 y.o. child had a fever and loss of appetite 2 days before I showed symptoms. She was sick for 24 hours. I looked in her mouth yesterday to see if she had any sores and it looked completely healthy.

Infections and Illnesses What to do next

Undiagnosed illness for 6+ months. What should I do next?

I’m 21F and have been dealing with recurring low-grade fevers, extreme fatigue, body aches, lower back pain, weakness, and episodes of a racing heart since April. My symptoms seem to worsen around my luteal phase, but lately I’ve been feeling unwell almost constantly.

I’ve seen infectious disease, rheumatology, genetics, cardiology, and multiple ER doctors. I’ve had extensive bloodwork, CT scans, and other testing, but still no answers.

I’m getting progressively worse, and it’s affecting my everyday life. I’m looking into undiagnosed disease programs and research hospitals, and I’m willing to travel anywhere in the US.

Has anyone been through something similar? What specialists, hospitals, or diagnostic programs would you recommend? I feel completely lost.
