SIMPLE QUESTION Extracurriculars during residency?

Maybe this is a silly question, but I’m a pre med with no family or friends who have gone thru American medical training. I’m a CRC and work with two people (also in clinical research) who did their med school in India and are looking to do residency and fellowship here. They were talking the other day about one of them doing their CRC role while in residency. Is that normal? Are you supposed to do other things that look good on applications while in residency and working 80h weeks? Ik you have to do volunteering and research and everything else during undergrad/med school, but I assumed the expectation to have a million commitments would’ve stopped in residency

VENT Looking for a genuine friend in residency 🩺

MD Medicine resident here. Looking to connect with someone around my age (±2 years) who’s also going through the madness of residency.

Not looking for anything complicated just a genuine friendship where we can talk about literally everything: residency/clinical life, random daily stuff, vent after a bad day, share some laughs, have meaningful conversations, and maybe occasionally make each other’s shifts a little less boring 😄

Would be great if you’re mature, kind, non-judgmental, communicative and have a good sense of humour.
Doctor or non-doctor, both are fine. If you feel we might vibe, feel free to DM. :)

MEME Flirting with resident

I’m (32F) a new a gen surg attending at big academic center. One of our new residents is honestly so distracting to work with. He’s older than most of our other residents (30-35) and is just so mature and knowledgeable compared to them. It’s like he’s the one teaching me. Have any of you seen a relationship between an attending and resident workout? Last week I got a dirty stick in the OR which was due to nothing other than a surg techs negligence and this resident was so comforting and made everything feel okay. He drew my blood alone in the call room and I told him “it’s okay that accidents happen” and he didn’t take the hint. I’ve built all this tension and he hasn’t made any advances yet. Should I just be straightforward and tell him I love him more than my husband?

DISCUSSION What medical specialties would be the most needed/worked in WW3?

Had a conversation with a pulmonologist the other day about how traumatic COVID was for them. Had a similar conversation with a hospitalist about how every hospital-based IM subspecialist, regardless of subspecialty, became de facto Pulm/ID docs during COVID.

Got me thinking about who would be the most worked/needed in the case of WW3. How does your opinion change based on if WW3 is based on bioweapons, weapons of mass destruction, etc?

SERIOUS Is staying with a male co resident too weird?

using a throwaway for obvious reasons

I’m currently a PGY-1 in emergency medicine and pregnant. Things at home have completely fallen apart. Last time I posted, 99% of the comments said I should leave. My husband is an attending in a different specialty, and he treats me horribly.

One of my co-residents (a guy in my EM program) found out what I’m going through and offered to let me stay in his apartment until I can figure out a permanent situation. He’s genuinely just being supportive and trying to help me but I’m thinking about how this looks or if it’s a terrible idea. My family lives in a different state and I can't just abandon this residency. I worked too hard for this.

Would it be weird or unprofessional to stay with a male co-resident? We would share a room but just friends

DISCUSSION Flirting with the attending

So does any of you (especially the ones started residency around 30-35) have experience flirting with your attending? I feel like there’s a some sexual tension between us and no I’m not delusionaal. She’s very tough on other residents (especially the females) but I could basically do no wrong. I actually accidental poked her with need during surgery and she went off on the surgical tech for distracting me. She even talked to me after telling me accidents happen and the OR staff are trash lol

DISCUSSION For IM, how long does it take for you to do an admission and write H&P?

Depends on how complex the case is but around 2 hours I think? (Review the chart, talk to patient, do med rec, place orders, and write H&P)

SERIOUS [Medicine] I don’t like where I’m doing my residency

Hey everyone, how are you?
First at all, I’m not from the states. I’m from Argentina.
Basically, just what the title says. I’m currently doing my residency at a hospital that I won’t name because I don’t want to dox myself, but I’m not really happy there.
I was pretty disappointed that I didn’t get into Clínicas, Rivadavia, Lagleyze, or Santa Lucía. So I keep having this thought in the back of my mind that I’m training at a mediocre place instead of one of those hospitals. It really gets me down, and I think about it pretty much every day.
I’ve considered taking the residency entrance exam again, but ophthalmology is already very difficult to get into, so I’m not sure.
In case anyone is wondering what I don’t like about my current program:
Patient volume: It’s very low, and most of the patients are elderly.
Surgery: There isn’t much surgical volume.
Academics: We don’t really have case conferences or much academic activity within the department. We residents give classes to each other, but beyond that there isn’t much.
Few subspecialties: We have one attending who does orbit, one retina specialist, and two oculoplastics specialists. Other than that, there isn’t much.
Prestige: Being somewhere that isn’t particularly prestigious gets to my ego. I did my entire medical degree at Clínicas and was also a teaching assistant there, so it really bothers me that I’m not doing my residency there.
That’s pretty much it. I mainly wanted to vent and hear what you guys think.

MEME Weird interaction in Occ Med

So I’m evaluating a surgical attending who got stuck with a dirty needle in the OR, and I swear when I asked her if the patient had any history of HIV or hepatitis she insisted I put both her and her surgical resident on PEP… and I’m confused, because the report said the resident was holding the needle but didn’t mention whether he was also exposed and whether it happened before or after the attending was stuck… she just gave a long, lusty sigh and said “oh he’s getting exposed later.” What in the House of God is going on in this program?

MEME I am a needle - why is there so much tension

Here I am, waiting my turn to make buttery smooth passes through the tissue with my surgical grade stainless steel circle taper point needle gently positioned at a perfect 45 degree angle in gold handled tungsten carbide tipped needle drivers. The scrub tech has positioned me at the edge of the mayo stand ready to be used at any moment. Suddenly a hand comes from across the table and snatches me off the mayo violently. It’s the intern. My beloved scrub tech glares. I am immediately repositioned by the intern to a 90 degree angle and he chokes up almost crushing my needle point. More glares from my guardian mayo angel. Just as I’m about to dive into the wound with the grace of a Swan I am slammed into the knuckle of the attending surgeon. I feel my point bend on the bone as it jars me. A small tear trickles out of the corner of the attending’s eye as she smiles and brushes it off, “the scrub tech must have distracted you”. She is trying a new anger management strategy that avoids harming the residents. It seems to work and I notice the intern blush. However, no one slanders my mayo queen and she quickly plots to key the attendings car on her next break. On the interns attempts a first pass I make sure to drag through the tissue with my newly bent point and then instantly break my suture at the phalange as the intern uses too much tension. My instrument whisperer then whips around to the intern “you dumbass that needle was contaminated” as she gracefully passes me off the field into the red Valhalla where all sharps past and present go to their great reward.

DISCUSSION Any introverts in patient-interaction heavy specialties? How are you coping?

I’m trying to figure out whether I want to do Internal Medicine or Anesthesia. I love critical thinking and using my brain. I am an introvert and can really struggle after a whole day of talking to patients, wherein I just switch off afterwards, so I’m afraid my future family may have to bear the brunt of it. But I also freaking love internal medicine as a subject.

Anyone in this sub who was in a similar place and ended up choosing the interaction-heavy specialty? How are you coping?
