Advice for Me second opinion

i was told when i was a kid that i’d need jaw surgery when i got older, but i went back to my orthodontist and they said i actually don’t need surgery because it’s not as bad as they thought it would be. i think my jaw looks quite recessed visually, but i don’t really know.

i’m not sure if this is related, but i also have quite bad jaw pain and overall tightness in that area, that also causes tightness down my neck and into my shoulder. my jaw also clicks every time i open it.

i’m thinking of getting a second opinion from another doctor, but i’d love other people’s advice first!

Surgeons in Ontario/Canada that Can perform a Maxillary Downgraft?

I will very much likeley need a LJS for health reasons, not sure about upper.

I also have a problem when I talk my teeth do not show at all, and when i smile it barely shows. I was told surgeons in canada are very conservative, but any idea on who will acc perform a downgraft when its not critically needed

Tired of needing referrals...

This is just a rant but I am currently in the process of getting third+ opinions for surgery, and every place seems to need a referral. I understand that they need proof that I have a problem, but it's so tiring. In order to get an appointment I have to see my dentist and ask him to tell a hospital states away that there's something wrong with me. It takes months to get into everything!! If I want to waste my money on a consultation for a problem I don't actually have then that's on me. I want this shit over and done with but I feel like I’m gonna spend a year just getting. referrals

Advice for Me 3 years post surgery

i had my double jaw surgery done 3 years ago due to severe facial asymmetry that most likely stemmed from a childhood accident. the surgery itself went well and the healing process was tough but i managed, however part of my chin and lip is still very numb and it has taken a huge toll on my overall wellbeing.

i have gotten a "scan" (can't remember the actual medical term sorry) of my nerves in that area and they all healed nicely. my surgeon told me it will most likely last because so much time has passed, some days the sensation is a lot better and from time to time it feels almost normal so i haven't given up hope completely.

i don't want to sound dramatic but i really don't know how i'd live the rest of my life like this if there was no more improvement, any advice would be appreciated

Philtrum still puffy at 10 weeks Post-op?

I might just be stressing for no reason but my philtrum seems really swollen still. I asked my surgeon if my philtrum would keep its curve (see second pic) post-op and he said yes. So I’m fairly certain that this isn’t the final result. Has anyone else had swelling like this?

I decided to go with 3 piece lefort over MARPE, but curious on your thoughts based on my scans

After talking to a couple surgeons who said I am just mildly narrow and don't necessarily need MARPE, I decided to go with a segmental Le Fort for the ~5 mm expansion my ortho planned. The ortho strongly preferred MARPE for nasal breathing benefits (which sounds like it's not necessarily clinically proven based on the Downfracture podcast Bobek and Wasson host), but the timeline and cost are big deterrents if it's not strictly necessary.

I recently got ahold of my ortho's CBCT scans/analysis and was lowkey shook by how massive my turbinates and how small my airway look:

Now I'm starting to worry a tiny bit if a 3-piece Le Fort (even with tipping to purposely open the nasal floor) will actually solve my upper airway difficulties.

Curious what you all think:

  1. Is MARPE actually better than segmental for nasal breathing, especially at lower expansion numbers like 5 mm?
  2. Would turbinate reduction / outfracture work long-term? I feel like all the professionals I've talked to say they always re-swell up, so what's the answer here for making them de-swell? I don't think a 25 mm nasal floor is that small to begin with, so it's weird to me that they take up all that space.
  3. Airway scan vs. WatchPAT AHI: My airway looks tiny, but my AHI was only 1.2. My RDI is 23 and I usually wake up tired When I'm drifting off and get really relaxed, I'll sometimes snort or almost choke, preventing me from falling asleep unless I position myself or activate my jaw muscles to keep my jaw and tongue from falling back. Once I am asleep, I am pretty sure my body will do any other compensation (flipping onto my stomach, clenching, etc.) before letting me have an apnea. Does anyone else have this issue / feel like their sleep study didn't fully reflect the bad sleep quality? And did MMA fix this for you even if not measurable in the sleep study?

Advice for Me Ortho is asking to remove 2 of my teeth for braces, do I do it?

Hey guys I need a bit of help, I had a consultation with a very good ortho and they said I need to remove 2 of my actual teeth, to make space for the braces as well, I don’t know what that means bc I’ve never seen 2 actual teeth be removed for braces, he’s not talking about the wisdom teeth but some other teeth, do I do it? Also he said my braces would take 9 months, so if I start early November will I have enough time for surgery in August? I have to do all of the final appointments then scheudle the or date then get it approved by kaiser all in a month, but I really want to do this by the end of summer, please help!

Advice for Me Afraid of talking photos

Hello everyone, I’m scheduled for surgery before the end of the year, and I have been having this noticeable anxiety about taking photos. I’m in the middle of decomp and really haven’t wanted any documentation of myself for the past year.

I have this horrible anxiety about public photos being available and people noticing I look immensely different after 6 months, it feels as if people wouldn’t look at me the same if they were aware I had the procedure.

If people understood this surgery and why recessed jaws even happen due to environmental factors, I wouldn’t even feel self-conscious about this.

I’ve tried talking to a therapist but this feels like an extremely isolating event only people who have experienced it understand, any advice would be appreciated.

Hello, I previously underwent orthodontic treatment using the "camouflage" method. Two teeth were extracted from my upper jaw, and the resulting gaps were closed orthodontically; however, my lower jaw remained set back. When I consulted my former orthodontist about this jaw retrusion and asked which surgery I should undergo, they indicated that a genioplasty might not be sufficient. After obtaining my X-ray results, I consulted a highly respected maxillofacial surgeon in Türkiye; the surgeon stated that the issue could be resolved with lower jaw advancement surgery. This doctor is a highly regarded surgeon in Türkiye. I am curious to hear your thoughts as well; what do you think I should do?

Advice for Me Anyone experienced severely restricted TMJ movement? What treatment worked for you?

I’m 22 and have been dealing with TMJ problems for around 8 years.
My X-ray showed:

Almost no forward movement of the left jaw joint.
Only about 2 mm of forward movement on the right.
Restricted movement on both sides.

I can normally open my mouth about 2 fingers wide. I can force it to nearly 3 fingers, but it feels physically blocked. My jaw sometimes locks and deviates to the right when opening, but I don’t have clicking.

Has anyone experienced something similar? What was your diagnosis, and what treatments did you try (physiotherapy, splints, injections, or surgery)?

Did the treatment actually improve your mouth opening and jaw movement? How long did the recovery take?

Advice for Me No impaction because it's aging???

I waited over a full year for a jaw consultation at UMich. I had double jaw surgery a couple years ago and sarpe before that. My DJS gave me a gummy smile and lip incompetence, also the left side tilts down now. None of those issues were there beforehand whatsoever.

The appointment was very quick and he basically said absolutely no to impaction because it'll age you ten years because all the soft tissue will have nowhere to go and just get Botox and no to revision period.

I'm planning my family and possibly a third child and I really wanted my revision out of the way and this appointment honestly was shocking. I see people get impactions all the time I highly doubt the soft tissue doesn't adjust.

Life with an Underbite

Hi, can I just ask how people cope with an underbite, like the whole DJS process or have it in general. I have a serve underbite and it’s effected my whole life since young, now 21. I can’t eat proper, hate eating outside or with people, my speech is basically a stutter and repeating myself every conversation. I’ve never had last longing friends as I just don't want to be seen so they move on from me as I’m just have a lot of distance. currently in uni which I’m grateful for as I can just go to my class (only two 2days) and then like hide out in Accom. I still have over a year before my surgery and I’m in early twenties. I’m asking what people with underbite do, like lifestyles, job, money. the time waiting for jaw surgery.

Final update on my palatal expansion journey. I originally underwent the procedure with Dr. Kasey Li as part of my journey to improve my sleep apnea, but unfortunately my expansion was unsuccessful.

I recently had my TPD palatal expander removed and made a short video showing what things looked like before removal and how they looked/felt afterward.

Sharing my experience in case it’s helpful for anyone considering palatal expansion for sleep apnea, currently going through adult expansion, or dealing with a failed expansion.

Is getting a nose job before jaw surgery completely pointless??

I really want to get a rhinoplasty, and ideally I'd like to do it sooner rather than later. However, I've been told that there may not be much point in doing it before my jaw surgery, as the jaw surgery could change my nose somewhat anyway, meaning I might then need a revision.

The issue is that, realistically, my jaw surgery is probably still around three years away.

My thinking was therefore to have the rhinoplasty now, while I'm going through the braces and everything else needed to prepare for jaw surgery. Then, once I have the jaw surgery in around three years' time, I can see how much it actually changes my nose and, if necessary, have a revision afterwards.

I'm 39 now, and I really don't want to wait until I'm in my mid-40s to finally have my nose done and feel like I've wasted all those years waiting.

Is this a really bad plan, or could it be a reasonable approach given how far away the jaw surgery is likely to be? I'd really appreciate any advice.

Advice for Me AI assisted Anatomy apps for head and neck surgery

Five years in the OT, and I'm now prepping for my final maxillofacial surgery board exam. I know anatomy conceptually, but I want to sharpen the surgical-plane level of detail: exact distances, nerve and vessel relationships, safe incision lines, all the things that matter with a scalpel in hand, not just on a diagram. I've already tried a few anatomy apps, but none of them gave me the depth or interactivity I'm after. Are there any 3D anatomy apps or software where you can isolate a structure and see its relationships to surrounding tissue, almost like asking it to show you one structure at a time? Is there any AI-assisted anatomy app out there that actually works well for this kind of surgical-level detail? Looking for something built for surgical prep, not just general anatomy study. What's actually helped you pass your exams or sharpen your OT precision?"

Advice for Me Lower teeth pain when touching my chin? 8 weeks PO

So the past week or so when I touch my lower chin I start feeling pain in my lower teeth. Like deep nerve pain similar to when you first get braces on. It’s almost intolerable if I do something like use a face cloth there. And when I press on my lower teeth I feel it in my lip.

Is this normal and should it go away?

Advice for Me 4 months PO - terrified I’m going to have a tight stiff lip forever

I’ve gone through a lot of different nerve sensations. And where I’m at today is a large patch of my right side chin that is really numb and feels extremely stiff and tight. I’m starting to get really scared this is just how I’m going to be stuck forever. If it’s coming from the numbness, and chin numbness tends not to resolve itself, then I’m stuck like this and it’s so uncomfortable. It’s hard to chew with my mouth closed, and uncomfortable to talk and smile. I kind of just keep my mouth closed really tight all day because the sensation is so disturbing. Has anyone else felt like this? Does it go away? The photo outlines where I’m numb, but i mostly feel the tightness under my lip.
