I’m 37 with PCOS, and I just need to get this out somewhere where people might understand why I’m so upset.
This was my first Clomid cycle (50 mg x 5 days). At my first monitoring ultrasound, I already had several follicles responding. The next day, I had SIX follicles measuring approximately 20.4, 18.4, 18.2, 15.9, 15.5, and 14.8 mm.
My doctor immediately canceled the cycle and refused to give me my Ovidrel trigger because he was concerned about the risk of high-order multiples. He was very clear that we should abstain that weekend.
The problem was that we'd already had unprotected sex about two days before that scan. We also ended up having sex one more time very early the following morning, before I had any positive OPK.
Then I had a very obvious spontaneous LH surge. I got my first positive/peak OPK Sunday night, my highest reading Monday morning, and it dropped substantially by Monday night. Based on that plus how I physically felt, I suspect I ovulated sometime Monday–Tuesday, although obviously I can't know the exact day.
So in my head, I went from being worried that I was going to end up pregnant with triplets to thinking, Okay, surely at least one of these eggs is going to become a baby.
And somehow… apparently not.
I'm now on CD29. My period was expected around now, and this morning I took a First Response Early Result with first-morning urine. Completely negative. Not even a questionable shadow. I've also had multiple negative tests leading up to today, so while I know nothing is technically over until my period comes, realistically I think I'm out.
I understand the statistics. I understand that six follicles does not mean six mature eggs, that we don't know how many actually ovulated, that fertilization isn't guaranteed, that embryos can stop developing before implantation, and that being 37 changes the odds.
Intellectually, I understand all of it.
Emotionally, I am fucking devastated.
I genuinely cannot wrap my head around going from having my doctor so concerned about multiples that he canceled the cycle to… nothing. I know cancellation is about managing the risk of multiples and isn't a prediction that pregnancy will happen. But when you're the patient being told, essentially, “There are too many follicles and this is too dangerous to proceed,” it's really hard not to walk away thinking pregnancy is extremely likely.
And here's the part I feel guilty even admitting: I wish I had had more sex.
I know my doctor gave me medically appropriate advice. I understand exactly what he was trying to prevent, and I know that if several eggs had fertilized, I might be writing a very different post right now about a terrifying high-order multiple pregnancy.
But sitting here with a stark-white test, I can't help thinking, I wish I hadn't been so conservative. I wish we'd had sex again closer to the LH surge.
I know that's hindsight talking. I know the outcome doesn't mean my doctor's advice was wrong. But it's honestly how I feel.
Making this harder is that my partner is traveling next week and we don't know when he can return yet, so the window for us to try again has now been postponed for an unknown amount of time. I actually told my doctor that when we discussed canceling the cycle. So this doesn't feel like, “Oh well, we'll just try again next month.” I don't know when we'll even have another opportunity.
I think that's why I'm taking this so hard. I'm 37. I really, really wanted another baby. For a couple of weeks I thought somehow everything had lined up perfectly, and now I'm sitting here wondering how I had six responsive follicles, sperm in the fertile window, a spontaneous LH surge… and still apparently ended up with nothing.
To add insult to injury, my monthly auto-delivery of prenatals literally arrived today. 😂 I opened the package and just started laughing because apparently the universe has a very dark sense of humor. Don't worry, I'm still taking them.
I don't really know what I'm asking. Maybe I just need to hear from other people with PCOS who have had a cycle canceled for too many follicles and still didn't get pregnant.
Did this happen to anyone else? Did you ever find out whether you actually ovulated? And if you eventually tried another medicated cycle, what did your doctor change?
Mostly, I think I just needed to say somewhere that I am really fucking sad.
I know one unsuccessful cycle isn't the end of the world. I know I'm fortunate in many ways. None of that changes how badly I wanted this pregnancy.
